RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2007-01813 INDEX CODE: 110.00 COUNSEL: NONE HEARING DESIRED: NO ________________________________________________________________ _ APPLICANT REQUESTS THAT: Her National Guard Bureau (NGB) Form 22, Report of Separation and Record of Service, be corrected to reflect her address as XXXXXX, XXXXX rather than XXXXXXX and that the Air Force Outstanding Unit Award (AFOUA) she was awarded be added to her NGB 22. ________________________________________________________________ _ APPLICANT CONTENDS THAT: The address shown on her NGB Form 22 is incorrect and an AFOUA she was awarded is not reflected on her NGB Form 22. In support of her appeal, the applicant has provided copies of her NGB Form 22, her discharge order, a decoration order and other paperwork to substantiate her address. Applicant’s complete submission, with attachments, is at Exhibit A. ________________________________________________________________ _ STATEMENT OF FACTS: Applicant began her military service on 18 June 1997. She was progressively promoted to the grade of staff sergeant (SSgt) with a date of rank (DOR) of 2 November 2002. She was honorably discharged from the Illinois Air National Guard (ILANG) on 15 February 2007 after serving for 10 years and 21 days. The applicant’s NGB Form 22 does, in fact, display the award of the AFOUA however the notation of the award is shown, not in Block 15 of the form, but in Block 18 as a continuation of Block 15. Therefore, no further action is required in regard to the AFOUA. ________________________________________________________________ _ AIR FORCE EVALUATION: The Air National Guard received this application for an advisory opinion on 11 June 2007 and has failed to provide one. ________________________________________________________________ _ THE BOARD CONCLUDES THAT: 1. The applicant has not exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of error or injustice. We did not address her request for the inclusion of the AFOUA on her NGB Form 22 because the AFOUA is shown on the NGB Form 22 in block 18 of that form. Regarding her change of address however, we did find that the applicant provided enough evidence to show the address on her NGB Form 22 was indeed, incorrect, and what her correct address should be. Therefore, we recommend that the records be corrected as indicated below. ______________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to APPLICANT be corrected to show that Block 19 of her NGB Form 22, reflects XXXXXXX rather than, XXXXXXXXX. ______________________________________________________________ The following members of the Board considered AFBCMR Docket Number BC-2007-01813 in Executive Session on 7 August 2008, under the provisions of AFI 36-2603: All members voted to correct the records, as recommended.
