The inclusive dates for his Air Force Achievement Medal (AFAM) be corrected. 2. He be issued a DD Form 256AF, Honorable Discharge Certificate. (Administratively Corrected). ________________________________________________________________ APPLICANT CONTENDS THAT: His records were recently corrected and he was issued a DD Form 215, Correction to DD Form 214, Certificate of Release or Discharge From Active Service. He would like item 5 corrected to reflect the correct inclusive dates of his AFAM. In support of his request, the applicant provides a personal statement, copies of his DD Form 214, DD Form 215, Special Order, dated 11 May 1993, his AFAM Citation, and correspondence from AFPC. The applicant’s complete submission, with attachments is at Exhibit A. ________________________________________________________________ STATEMENT OF FACTS: The applicant served on active duty in the Regular Air Force from 15 Aug 1990 to 5 Jul 1995 for a total of 4 years, 10 months, and 21 days. The AFAM is awarded to members of the Armed Forces of the United States and foreign military personnel, below the rank of 0-6, after 20 Sep 1981, who while serving in any capacity with the U.S. Air Force, distinguish themselves by meritorious service or achievement. The remaining relevant facts pertaining to this application are described in the letter prepared by the Air Force office of primary responsibility at Exhibit B. ________________________________________________________________ AIR FORCE EVALUATION: HQ AFPC/DPSIDRA recommends denial. DPSIDR states the applicant appears to base his request on the letter he received from the Directorate of Assignments (DPAPP), dated 28 Apr 2011, in which they verified his service in Saudi Arabia from 31 Dec 1990 to 5 Apr 1991.However, the applicant has a signed certificate/citation and Special Order, dated 11 May 1993, authorizing him entitlement to the AFAM with an inclusive period of 24 Aug 1990 to 13 Mar 1991. In a prior request, the applicant provided a copy of a travel order which states he was deployed on or about 30 Dec 1990, in support of Operation DESERT SHIELD; however, the travel voucher verifying the travel order, including exact arrival and departure dates, was not provided or located. Although DPAPP verified the applicant’s service in Saudi Arabia from 31 Dec 1990 to 5 Apr 1991, they have been unable to locate any documentation verifying the recommending official changed the applicant’s AFAM inclusive period to the dates verified by DPAPP. The complete DPSIDRA evaluation is at Exhibit C. ________________________________________________________________ APPLICANT'S REVIEW OF AIR FORCE EVALUATION: On 23 Mar 2012, a copy of the Air Force evaluation was forwarded to the applicant for review and comment within 30 days. To date, a response has not been received (Exhibit D). ________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was not timely filed; however, it is in the interest of justice to excuse the failure to timely file. 3. Insufficient relevant evidence has been presented to demonstrate the existence of error or injustice. We took notice of the applicant's complete submission in judging the merits of the case; however, we agree with the opinion and recommendation of the Air Force office of primary responsibility and adopt its rationale as the basis for our conclusion the applicant has not been the victim of an error or injustice.
