RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2011-04361 COUNSEL: NONE HEARING DESIRED: NO _________________________________________________________________ APPLICANT REQUESTS THAT: Her 29 Sep 11 Fitness Assessment (FA) be removed from the Air Force Fitness Management System (AFFMS). _________________________________________________________________ APPLICANT CONTENDS THAT: She received an unsatisfactory score on her FA due to the medication (Lisinopril-HCTZ) she was prescribed for her high blood pressure. Although she was experiencing dizziness prior to her FA, she did not indicate such on the fitness screening questionnaire (FSQ); however, she did ask the personnel at the testing office if she should test and was advised not to test until she was seen by her primary care manager (PCM). She then contacted her commander and Unit Fitness Program Manager (UFPM) and was told that she was not on a medical profile that exempts her from completing the walk. She decided to take the test to avoid having a late test date. She further notes that after receiving the unsatisfactory score, she discussed her medical concerns with the commander and UFPM. Her commander then advised her to seek medical care. In support of her request, the applicant provides copies of documents extracted from her military personnel and service medical records. The applicant's complete submission, with attachments, is at Exhibit A. _________________________________________________________________ STATEMENT OF FACTS: The applicant is currently serving in the Regular Air Force in the grade of master sergeant (E-7). The remaining relevant facts pertaining to this application are contained in the letter prepared by the appropriate office of the Air Force, which is attached at Exhibit B. _________________________________________________________________ AIR FORCE EVALUATION: HQ AFPC/DPSIM recommends denial, noting that Air Force Instruction (AFI) 36-2905, Fitness Program, states the service member must complete the FSQ prior to the FA. The medical provider must evaluate all service members with health issues noted on the FSQ prior to the FA. The applicant completed the FSQ prior to her FA, but did not identify her medical concerns on the FSQ. If the applicant had indicated her concerns on the FSQ, a medical provider would have evaluated her for those concerns prior to the FA. Also, although the applicant’s medication has been confirmed as possibly causing the increase in her heart rate and dizziness, the medication is not identified in the governing instruction as a medication affecting FA participation. The complete AFPC/DPSIM evaluation, with attachments, is at Exhibit B. _________________________________________________________________ APPLICANT'S REVIEW OF AIR FORCE EVALUATION: She completed the FSQ prior to her FA and indicated she was experiencing light headedness, dizziness, and blackouts. The Fitness Assessment Cell (FAC) did not want her to test due to what she indicated on the questionnaire. Her commander, who is a medical doctor and UFPM felt that since she had previously tested with the medical condition (heart murmur), that it would be alright for her to test. In order for her to take the FA she had to complete another FSQ indicating she was not having any problems. The original FSQ was shredded by FAC personnel. However, no one realized it was the medication causing her low blood pressure and increased heart rate. Although, the medication is not listed as a medication affecting FA participation, it should be on list indicating the heart rate could be affected. She has since been prescribed a different medication and has been able to pass her FA with no problems. The applicant’s complete response, with attachments, is at Exhibit D. _________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of error or injustice. After a thorough review of the evidence provided by the applicant, to include the supporting statement from her Family Practice Physician Assistant, we believe she has raised sufficient doubt that her medication adversely impacted her performance during the contested fitness assessment. Therefore, to preclude the possibility of an injustice to the applicant, we recommend that her records be corrected as indicated below. _________________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to the APPLICANT, be corrected to show that her Fitness Assessment, dated 29 September 2011, be removed from the Air Force Fitness Management System (AFFMS) database. _________________________________________________________________ The following members of the Board considered AFBCMR Docket Number BC-2011-04361 in Executive Session on 5 Jun 12, under the provisions of AFI 36-2603: All members voted to correct the record as recommended.
