RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2012-04796 COUNSEL: NONE HEARING DESIRED: NO ________________________________________________________________ APPLICANT REQUESTS THAT: His Fitness Assessment (FA), dated 20 April 2011, be removed from the Air Force Fitness Management System (AFFMS). ________________________________________________________________ APPLICANT CONTENDS THAT: He suffered a back injury during the contested FA. He turned in all of the required paperwork to his Fitness Program Manager; however, he was not given a chance to retest. The applicant’s complete submission, with attachments, is at Exhibit A. ________________________________________________________________ STATEMENT OF FACTS: The applicant failed the FA dated 20 April 2011. ________________________________________________________________ AIR FORCE EVALUATION: AFPC/DPSIM recommends the applicant be exempt from the cardio, pushup and sit-up components of the FA, dated 20 April 2011. The overall composite score will change to reflect 100 (Excellent). DPSIM noted, the applicant provided an AF Form 469, Duty Limiting Report, dated 26 April 2011 stating no lifting, pushing pulling greater than 40 pounds, run at own pace, and no repetitive bending of the waist. The complete DPSIM evaluation, with attachments, is at Exhibit B. ________________________________________________________________ APPLICANT'S REVIEW OF THE AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 9 November 2012 for review and comment within 30 days. As of this date, no response has been received by this office (Exhibit C). ________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of error or injustice. While we note the applicant requests to have the entire FA removed, the medical documentation only warrants an exemption to the cardio, sit-up and push-up components. Therefore, we agree with the opinion and recommendation of the Air Force office of primary responsibility to grant partial relief, which will result in an overall composite score of 100 (Excellent). Therefore, we recommend the applicant’s record be corrected as indicated below. ________________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to APPLICANT be corrected to show that the cardio, push-up, and sit-up components of the Fitness Assessment, dated 20 April 2011, reflect “exempt” in the Air Force Fitness Management System. ________________________________________________________________ The following members of the Board considered AFBCMR Docket Number BC-2012-04796 in Executive Session on 16 July 2013, under the provisions of AFI 36-2603: All members voted to correct the records, as recommended.
