APPLICANT REQUESTS THAT:

His 21 Dec 2011 Fitness Assessment (FA) failure be corrected in the Air Force Fitness Management System (AFFMS).

APPLICANT CONTENDS THAT:

He was tested in a component that his AF Form 422, Notification of Air Force Member’s Qualification Status, and AF Form      469,  Duty Limiting Condition Report, exempted him from and received a below minimum score.   

In support of his request, the applicant provides communique with his first sergeant, copies of his AF Form 469, AF Form   422 and a letter from his medical provider.

AIR FORCE EVALUATION:

AFPC/DPSIM recommends partial approval.  The applicant received an AF Form 469 from his primary medical care provider stating  he had a medical condition preventing him from passing the cardio component of the FA effective 18 Nov 2011.  Therefore, the applicant should not have been required to complete the cardio portion of the FA.  Since his medical condition did not restrict him from the push-up, sit-up and abdominal circumference components of the FA, there is no authority to remove these portions from the AFFMS.  DPSIM recommends exempting the applicant from the cardio component of the FA which would result in a composite score of 86.00, Satisfactory.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

On 29 Oct 2013, a copy of the Air Force evaluation was forwarded to the applicant for review and comment within 30 days (Exhibit C).  As of this date, this office has not received a response.

THE BOARD RECOMMENDS THAT:

1.  The applicant has exhausted all remedies provided by existing law or regulations.

2.  The application was timely filed.

3.  Sufficient relevant evidence has been presented to demonstrate the existence of error or injustice.  After carefully reviewing this application, we agree with the opinion and recommendation of the Air Force office of primary responsibility and adopt the rationale expressed as the basis for our conclusion that the applicant has been the victim of an error or injustice.   We recommend the applicant’s records be corrected as indicated below.

THE BOARD DETERMINES THAT:

The pertinent military records of the Department of the Air Force relating to APPLICANT be corrected to show that the cardio  component of his Fitness Assessment, dated 21 Dec 2011, be amended to reflect “exempt” in the Air Force Fitness Management System.
