His DD Form 214, Certificate of Release or Discharge from Active Duty, issued in conjunction with his 31 May 90, separation for retirement, Block 12a, Date Entered Active Duty This Period be changed to May 70 versus 14 Jul 77. 2. He receive a copy of all his military and medical records. _________________________________________________________________ APPLICANT CONTENDS THAT: His DD Form 214 has his entry date of active duty as 14 Jul 77. The net active service this period is 12 years, 10 months and 17 days. His entry date is May 70 and his retirement date is 1 Jun 90. In support of his request, the applicant provides a copy of his DD Form 214. The applicant's complete submission, with attachment, is at Exhibit A. _________________________________________________________________ STATEMENT OF FACTS: On 5 May 70, the applicant enlisted in the Regular Air Force. He was issued DD Forms 214 for the following periods: 5 May 70 – 2 Aug 73; 3 Aug 73 – 13 Jul 77; and 14 Jul 77 – 31 May 90. He is credited with 20 years, 2 months and 12 days of active service. _________________________________________________________________ AIR FORCE EVALUATION: AFPC/DPSIPV recommends denial. The applicant should have received a DD Form 214 for his prior active duty service. However, his military personnel record was not available from the National Personnel Record Center; therefore, DPSIPV was unable to verify whether the applicant’s service was continuous active duty from May 70. The complete DPSIPV evaluation is at Exhibit C. _________________________________________________________________ APPLICANT’S REVIEW OF THE AIR FORCE EVALUATION: He feels a typing error was made because he entered military duty on 5 May 70 and has evidence to prove and support his request. In further support of his appeal, the applicant provided copies of his DD Forms 214 issued in conjunction with his 2 Aug 73, 13 Jul 77, and 31 May 90 separations, Retirement Orders, Statement of Service, and various other documents associated with his request. The applicant’s complete submission, with attachments, is at Exhibit D. _________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was not timely filed; however, it is in the interest of justice to excuse the failure to timely file. 3. Insufficient relevant evidence has been presented to demonstrate the existence of an error or injustice. The applicant requests his DD Form 214, issued in conjunction with his 31 May 90, separation for retirement, Block 12a, be changed to reflect 5 May 70; however, this date is properly recorded on his DD Form 214 issued in conjunction with his 2 Aug 73 separation. As such, we find no basis to disturb the record. Regarding the applicant’s requests for copies of his military and medical records, the AFBCMR is not the custodian for these records. However, the applicant may obtain a copy by submission of a written request to the National Personnel Records Center, 1 Archives Drive, St. Louis, Missouri 63138.
