RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2014-00689 COUNSEL: NONE HEARING DESIRED: NO ________________________________________________________________ APPLICANT REQUESTS THAT: His DD Form 214, Certificate of Release or Discharge from Active Duty, Block 10, Servicemembers’ Group Life Insurance (SGLI) Coverage, be changed from $200,000.00 to $400,000.00. ________________________________________________________________ APPLICANT CONTENDS THAT: He believes block 10, needs to be changed to the current amount of coverage of $400,000.00. The applicant’s complete submission, with attachments, is at Exhibit A. ________________________________________________________________ STATEMENT OF FACTS: The applicant is a former member of the Air Force who retired on 1 November 2013 in the grade of master sergeant. ________________________________________________________________ AIR FORCE EVALUATION: AFPC/DPFC recommends approval. The applicant has provided a leave and earnings statement (LES) from the Defense Accounting Service (DFAS) reflecting a premium deduction for SGLI in the amount of $27.00, which is the premium for coverage of $400,000.00. A copy of the applicant’s digital LES from October 2013 was obtained from DFAS and reflects the premium deductions for $27.00. The applicant’s Servicemembers’ Group Life Insurance Election and Certificate, SGLV 8286, obtained from his automated personnel record reflects coverage for $400,000.00. There is no record he applied for a request for the accelerated benefit option. A complete copy of the AFPC/DPFC evaluation, with attachments, is at Exhibit C. ________________________________________________________________ APPLICANT'S REVIEW OF AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 22 September 2014 for review and comment within 30 days (Exhibit D). As of this date, no response has been received by this office. ________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of an injustice. We took notice of the applicant's complete submission in judging the merits of the case and agree with the opinion and recommendation of the Air Force office of primary responsibility and adopt its rationale as the basis for our conclusion that relief should be granted. Therefore, we recommend the applicant’s record be corrected as indicated below. ________________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to the APPLICANT be corrected to show that the DD Form 214, Certificate of Release or Discharge from Active Duty, Block 10, SGLI Coverage, issued in conjunction with his 31 October 2013 retirement, be corrected to reflect the amount as $400,000, rather than $200,000.
