RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2014-00910 COUNSEL: NONE HEARING DESIRED: NO APPLICANT REQUESTS THAT: His Home of Record (HOR) be corrected to reflect Yokota Air Base, Japan. APPLICANT CONTENDS THAT: His DD Form 4/1, Enlistment / Reenlistment Document, correctly identifies his HOR; however, when his Air Force Reserve Officer Training Corps Detachment created his Extended Active Duty (EAD) Order, they failed to input his HOR. In support of his request, the applicant submits his DD Form 4/1 and EAD Order. The applicant’s complete submission, with attachments, is at Exhibit A. STATEMENT OF FACTS: The applicant initially entered the Air Force Reserve on 1 Sep 04. His DD Form 4/1, Enlistment / Reenlistment Document listed Yokota Air Base, Japan as his HOR. The applicant entered the Regular Air Force on 30 Apr 06. On 30 Apr 14, the applicant was furnished an Honorable discharge, and was credited with 8 years and 1 day of active service. The applicant’s DD Form 214, Certificate of Release or Discharge from Active Duty, dated 30 Apr 14 listed Tallahasse, Florida as his HOR. The remaining relevant facts pertaining to this application, extracted from the applicant’s military records, are described in the memoranda prepared by the Air Force offices of primary responsibility (OPR), which are included at Exhibit C. AIR FORCE EVALUATION: AFPC/DPSIPS recommends approval. After reviewing the applicant’s military personnel record it does in fact reveal his HOR was Yokota Air Base, Japan. A complete copy of the AFPC/DPSIPS evaluation is at Exhibit C. APPLICANT'S REVIEW OF AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 22 Sep 14 for review and comment within 30 days (Exhibit D). As of this date, no response has been received by this office. THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice. We took notice of the applicant's complete submission in judging the merits of the case and agree with the opinion and recommendation of the Air Force office of primary responsibility (OPR) and adopt its rationale as the basis for our conclusion the applicant has been the victim of an error. Therefore, we recommend the applicant's records be corrected as indicated below. THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to the applicant be corrected to show that his Home of Record reflect “Yokota Air Base, Japan”.
