The North Atlantic Treaty (NATO) medal. (ADMINSTRATIVELY CORRECTED) The Nuclear Deterrence Operations Service medal. (ADMINISTRATIVELY CORRECTED) APPLICANT CONTENDS THAT: He received the AFCM while stationed at Incirlik Air Base, Turkey; however, it is not reflected in his records due to an administrative error. The applicant’s complete submission, with attachments, is at Exhibit A. STATEMENT OF FACTS: The applicant is former member of the Air Force who retired on 31 December 2011 in the grade of master sergeant. Upon review of his records, AFPC/DPSID verified his entitlement to the North Atlantic Treaty medal and the Nuclear Deterrence Operations Service medal. His records will be corrected upon final board action. AIR FORCE EVALUATION: AFPC/DPSID recommends approval. The AFCM was authorized by the Secretary of the Air Force on 28 March 1958 and is awarded to members of the Armed Forces of the United States, below the grade of colonel and foreign military personnel, who, while serving in any capacity with the Department of the Air Force, distinguish themselves by outstanding achievement or meritorious service. The applicant provided what appear to be an authentic citation and a signed certificate for the award of the AFCM with second Oak Leaf Cluster for the inclusive period of 15 March 1997 to 5 June 1998. However, no Special Order was provided with this request or could be located within his official military personnel record. The Special Order officially authorizes award of a decoration and without this, the applicant’s entitlement could not be verified. The applicant's DD Form 214, Certificate of Release or Discharge from Active Duty, dated 31 December 2011, reflects award of the AFCM with four Oak Leaf Clusters. The AFCM with four Oak Leaf Clusters is currently on the applicant's DD Form 214; however, in order to provide the relief sought, the applicant will need a Special Order for the inclusive period from 15 March 1997 to 5 June 1998. The complete DPSID evaluation is at Exhibit C. APPLICANT'S REVIEW OF AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 31 July 2015 for review and comment within 30 days. As of this date, no response has been received by this office (Exhibit D). THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of error or injustice. Although the applicant was unable to provide a copy of the Special Order awarding him the AFCM, we find the documentation provided sufficient to conclude he was awarded the AFCM, second Oak Leaf Cluster. Therefore, we agree with the Air Force office of primary responsibility that the certificate and citation provided appear to be authentic and recommend, in addition to the administrate corrections noted by the OPR, the record be corrected as indicated below. THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to the APPLICANT be corrected to show that on 30 March 1998, he was awarded the Air Force Commendation Medal, second Oak Leaf Cluster, for meritorious service for the period of 15 March 1997 to 5 June 1998.
