IN THE MATTER OF: DOCKET NUMBER: BC-2015-00740 COUNSEL: NONE HEARING DESIRED: YES APPLICANT REQUESTS THAT: His DD Form 214, Certificate of Release or Discharge from Active Duty, for the period 1 Jul 93 to 29 May 97 be amended in Section 14, Military Education to add USAFA Survival, Evasion, Resistance, and Escape (SERE) training. APPLICANT CONTENDS THAT: He completed the training in the summer of 94 and it was omitted from his DD Form 214. The applicant’s complete submission, with attachments, is at Exhibit A. STATEMENT OF FACTS: On 1 Jul 93, the applicant entered the United States Air Force Academy (USAFA). On 29 May 97, the applicant was medically disenrolled from the USAFA. According to the documentation provided by applicant, while he was attending the USAFA he completed SERE training in the summer of 1994. The remaining relevant facts pertaining to this application are described in the memorandum prepared by the Air Force office of primary responsibility (OPR), which is at Exhibit C. AIR FORCE EVALUATION: USAFA/A1A recommends granting relief noting the applicant completed the SERE training in the summer of 94 while attending the USAFA. A1A states that under the provisions of AFI 36-3202, Separation Documents, formal in-service training courses completed during continuous active military service are listed on the DD Form 214. When the applicant’s DD Form 214 was issued, there was an administrative error which omitted his completion of the SERE training. A complete copy of the USAFA/A1A evaluation, with attachment, is at Exhibit C. APPLICANT'S REVIEW OF AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 28 Apr 15 for review and comment within 30 days (Exhibit E). As of this date, no response has been received by this office. THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was timely filed. 3. Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice. We took notice of the applicant's complete submission in judging the merits of the case and agree with the opinion and recommendation of the Air Force office of primary responsibility (OPR) and adopt its rationale as the basis for our conclusion the applicant has been the victim of an error or injustice. Therefore, we recommend the applicant's records be corrected as indicated below. 4. The applicant’s case is adequately documented and it has not been shown that a personal appearance with or without counsel will materially add to our understanding of the issues involved. Therefore, the request for a hearing is not favorably considered. THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to the APPLICANT be corrected to show that the DD Form 214, Certificate of Release or Discharge from Active Duty, issued in conjunction with his 29 May 1997 separation, be amended in Item 14, Military Education, to include (USAFA) Survival, Evasion, Resistance, and Escape (SERE) Training.
