RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS. IN THE MATTER OF: DOCKET NUMBER: BC-2015-01881 COUNSEL: NONE HEARING DESIRED: NO APPLICANT REQUESTS THAT: His Air Force Commendation Medal (AFCM) be corrected. APPLICANT CONTENDS THAT: His AFCM reflects a name other than his own on the reverse side. It did not seem important at the time he received the medal; however, since he has become older it is more important to him to be corrected. The applicant’s complete submission, with attachments, is at Exhibit A. STATEMENT OF FACTS: The applicant initially entered the Regular Air Force on 10 May 1966. On 29 January 1969, the applicant was furnished an honorable discharge, and was credited with 2 years, 8 months, and 20 days of active service. On 6 October 2015, AFPC/DPSIDR was able to verify the applicant’s entitlement to the following Air Force Medals and/or Ribbons and will correct his records administratively: a. Air Force Outstanding Unit Award (AFOUA). b. Air Force Good Conduct Medal (AFGCM). c. Republic of Vietnam Gallantry Cross with Palm (RVNGC w/P). d. Vietnam Service Medal with four Bronze Service Stars (VSM w/4BSS) instead of VSM w/1BSS. The remaining relevant facts pertaining to this application are contained in the memorandum prepared by the Air Force office of primary responsibility (OPR), which is attached at Exhibit C. AIR FORCE EVALUATION: AFPC/DPSID recommends denial. The Air Force Commendation Medal is awarded to members of the Armed Forces of the United States, below the grade of Colonel and foreign military personnel, who, while serving in any capacity with the Department of the Air Force after 28 March 1958, distinguish themselves by outstanding achievement or meritorious service. After a thorough review of the applicant’s official military personnel records, verification could not be made of the applicant’s award of the AFCM. Although the applicant provided a copy of an AFCM citation, AFPC/DPSID was not able to locate the citation, certificate, or Special Order for award of an AFCM. The Special Order is the official source documentation to verify award of the AFCM. Without the Special Order, verification of the award could not be confirmed. A complete copy of the AFPC/DPSID evaluation is at Exhibit C. APPLICANT'S REVIEW OF AIR FORCE EVALUATION: A copy of the Air Force evaluation was forwarded to the applicant on 6 November 2015 for review and comment within 30 days (Exhibit D). As of this date, no response has been received by this office. THE BOARD CONCLUDES THAT: 1.  The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was not timely filed; however, it is in the interest of justice to excuse the failure to timely file. 3.  Insufficient relevant evidence has been presented to demonstrate the existence of an error or injustice. We took notice of the applicant’s complete submission in judging the merits of the case; however, we agree with the opinion and recommendation of the Air Force office of primary responsibility (OPR) and adopt its rationale as the basis for our conclusion the applicant has not provided sufficient evidence such as the signed certificate or Special Order for award of an AFCM. We note that AFPC/DPSIDR has verified the applicant’s entitlement to the AFOUA, AFGCM, RVNGC w/P, and VSM w/4BSS and his records will be corrected administratively. Therefore, in the absence of evidence to the contrary, we find no basis to recommend granting any relief beyond that rendered administratively. THE BOARD DETERMINES THAT: The applicant be notified the evidence presented did not demonstrate the existence of material error or injustice; the application was denied without a personal appearance; and the application will only be reconsidered upon the submission of newly discovered relevant evidence not considered with this application.
