The applicant requests that his DD Form 214 (Report of Separation From Active Duty) be changed to reflect that he successfully completed twelve years of high school and that his Social Security Number (SSN) should be corrected to read ……. 3. He was born on 18 August 1957. The applicant’s enlistment contract and other legal documents shows that he completed 11 years of formal education. On 6 July 1976, he enlisted in the Regular Army for 3 years. The applicant’s enlistment contract and his entire military personnel file shows his SSN as ……. The applicant completed basic training. The highest grade he achieved was pay grade E-2. 4. On 24 November 1976, the applicant accepted nonjudicial punishment under Article 15, Uniform Code of Military Justice, for the wrongful possession of Marijuana. His imposed punishment was a reduction to pay grade E-1 and a forfeiture of $75 pay per month for 2 months. 5. On 29 March 1977, the applicant was discharged, in pay grade E-2 under the provisions of Army Regulations 635-200, paragraph 5, for failure to maintain acceptable standards for retention (Expeditious Discharge Program) with a general discharge. He had completed 8 months and 24 days of creditable active service. CONCLUSIONS: 1. The evidence of record clearly shows that the applicant’s SSN is … and not ……………. 2. The applicant also alleges that he completed 12 years of formal education. However, there is no evidence in support of this allegation. The applicant’s enlistment contract and other legal documents shows that he only completed 11 years of formal education at the time of enlistment. 3. Therefore, in view of the foregoing, it would be appropriate to correct the record, but only as recommended below. RECOMMENDATION: 1. That all of the Department of the Army records related to this case be corrected by issuing the individual concerned a DD Form 215 (Correction to the DD Form 214), by showing that his SSN is …… instead of ……. 2.
