2. The applicant requests correction of his military records to reflect award of a third Army Achievement Medal and completion of the Combat Lifesavers Course and German Headstart. He states the information was omitted from his separation document. 3. Records available to the Board indicate the applicant enlisted and entered active duty on 29 February 1988. He arrived in Germany in June 1988 and completed an 80 hour German Headstart Course in July 1988. Prior to his departure from Germany in June 1990 he was awarded two Army Achievement Medals, one for the period 29 October 1989 through 17 November 1989 and the other for the period 19 June 1988 through 6 June 1990. 4. In June 1990, upon his return to the United States he was assigned to Fort Hood, Texas where he completed a 40 hour Combat Lifesaver Course and was awarded his third Army Achievement Medal. 5. On 11 July 1991 the applicant was discharged from active duty as a result of physical disability and received severance pay. His DD Form 214 reflects award of only two Army Achievement Medals and does not reflect completion of either the German Headstart or Combat Lifesaver courses. 6. Army Regulation 635-5 establishes the policies and procedures for completion and distribution of the DD Form 214. In pertinent part it states that item 14 (military education) will list formal in service training courses successfully completed, during the period of service covered on the separation report, by title, length in weeks, and month and year completed. This information is to assist the soldier after separation in job placement and counseling. CONCLUSIONS: 1. The evidence confirms the applicant was awarded three Army Achievement Medals while on active duty although on two are reflected on his separation document. 2. The evidence also confirms he completed a Combat Lifesaver Course and Germany Headstart Course which meet the requirements for entry on his separation document. 3. In view of the foregoing, it would be appropriate to correct the applicant’s records as recommended below. RECOMMENDATION: That all of the Department of the Army records related to this case be corrected: a. By showing that the individual concerned was awarded three Army Achievement Medals; and b. By showing that he completed a 2 week German Headstart Course in July 1989 and a 1 week Combat Lifesaver Course in June 1990.
