Exhibit B - Military Personnel Records (including advisory opinion, if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to reflect her overseas duty location; and that she be issued the Army Commendation Medal and the certificate reflecting that award. 2. The applicant states that her DD Form 214 does not show where she served overseas. She was not issued the Army Commendation Medal or the certificate for that award. 3. The applicant provides a copy of her DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant entered on active duty on 23 April 2002. Her official military personnel file (OMPF) shows that she trained at Fort Leonard Wood, Missouri, and was assigned to Fort Polk, Louisiana thereafter. 2. The applicant was discharged on 4 November 2003 at Fort Polk because of pregnancy. Her DD Form 214 shows that she had 3 months and 10 days of foreign service, but does not indicate where she served. Her OMPF does not contain any evidence of her foreign service. 3. The applicant’s DD Form 214 shows that she was awarded the Army Commendation Medal. There is no evidence whether or not she was issued the Army Commendation Medal or the certificate reflecting that award. 4. Army Regulation 600-8-22 provides information regarding the original issue or replacement of Army medals, and states that requests will be honored from the original recipient of the award. Requests should be directed to the National Archives Records Administration, ATTN: NRPMA-M, 9700 Page Avenue, St. Louis, Missouri 63512-5200. In view of this, the applicant’s request for issue of the Army Commendation Medal will not be further addressed. DISCUSSION AND CONCLUSIONS: 1. There is no evidence and the applicant has not provided any, to show where she served overseas. Consequently, her request to correct her DD Form 214 to show where she served cannot be granted. 2. There is no evidence and the applicant has not submitted any to show whether or not she was issued an Army Commendation Medal or the accompanying certificate. Nevertheless, a certificate could not be issued without pertinent information, dates of the award, etc. Consequently, her request for award of an Army Commendation Medal Certificate cannot be granted.
