Exhibit B - Military Personnel Records (including advisory opinion, if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant, as the daughter of a deceased former service member (FSM), requests, in effect, that item 6 (Organization) on her late father’s WD AGO Form 53-55 (Enlisted Record and Report of Separation) be corrected to show the 475th Quartermaster Remount Troop instead of the 476th Quartermaster Remount Troop. 2. The applicant states, in effect, that it was brought to her attention that the FSM was a member of the 475th Quartermaster Remount Troop by a former member of the 475th Quartermaster Remount Troop. This former member states that he was with the FSM in the service and saw him every day for three years. 3. The applicant provides a copy of the FSM’s WD AGO Form 53-55. CONSIDERATION OF EVIDENCE: 1. The FSM's military records are not available to the Board for review. A fire destroyed approximately 18 million service members’ records at the National Personnel Records Center in 1973. It is believed that the FSM's records were lost or destroyed in that fire. However, there were sufficient documents remaining in a reconstructed record for the Board to conduct a fair and impartial review of this case. 2. The FSM was inducted on 17 March 1943 and entered active duty on 24 March 1943. He served in the Asiatic-Pacific Theater of Operations from 23 November 1944 until approximately 14 March 1946 and was honorably discharged on 19 March 1946. 3. Item 6 on the FSM's WD AGO Form 53-55 shows he was assigned to the 476th Quartermaster Remount Troop. 4. There is no evidence of record which shows the FSM was assigned to the 475th Quartermaster Remount Troop. 5. A DD Form 215 (Correction to DD Form 214), dated 12 February 2007, amended the FSM’s WD AGO Form 53-55 to show the Asiatic-Pacific Campaign Medal with two bronze service stars, the Good Conduct Medal, and the World War II Victory Medal. DISCUSSION AND CONCLUSIONS: There is no evidence of record which shows the FSM was assigned to the 475th Quartermaster Remount Troop during World War II. Therefore, there is insufficient evidence on which to amend item 6 on his WD AGO Form 53-55.
