Exhibit B - Military Personnel Records (including advisory opinion, if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that the Southwest Asia (SWA) deployment dates annotated on her Correction to DD Form 214 (DD Form 215), dated 14 March 2005, 13 February 1991 through 15 May 1991, were incorrect and should be corrected to show she was deployed from 9 December 1990 through June 1991. 2. The applicant states, in effect, that the dates of deployment to SWA in support of Desert Storm/Shield reflected on her DD Form 215 are incorrect. She claims she was stationed in Bamberg, Germany, and was assigned to the 504th Maintenance Company, when she deployed to SWA for approximately 186 days during the period 19 December 1990 through June 1991. She further claims that she was put on a unit manifest; however, has no orders for this deployment. She further states that she is requesting her deployment dates to SWA be corrected so that her Foreign Service dates are accurate. 3. The applicant provides the following documents in support of his application: Separation Document (DD Form 214); Corrections to DD Form 214 (DD Forms 215) dated 9 April 1997 and 14 March 2005; Joint Force Headquarters New Jersey, Memorandum, dated 29 April 2005; Statement of Medical Examination and Duty Status; Department of Military and Veterans Affairs Orders Number 105-320, dated 14 April 2004; Department of Military and Veterans Affairs, Orders Number 122-304, dated 2 May 2003; Sworn Statement (DA Form 2823); Rights Warning Procedure/Waiver Certificate (DA Form 3881); Reserve Component Medical Cover Sheet; Personal Data Sheet (SF 600); and Disability Counseling Statement. CONSIDERATION OF EVIDENCE: 1. The applicant’s record shows she enlisted into the Regular Army and entered active duty on 14 May 1985. She was trained in and awarded military occupational specialty (MOS) 52D (Power Generator Equipment Repairer). 2. The applicant's Official Personnel Military File (OMPF) is void of any orders or other documents regarding her deployment to SWA in support of Desert Shield/Storm. 3. On 24 June 1991, the applicant was honorably released from active duty after completing a total of 6 years, 1 month, and 11 days of active military service. The DD Form 214 she was issued at the time shows she earned the following awards during her active duty tenure: Army Service Ribbon; Overseas Service Ribbon with Numeral 2; National Defense Service Medal; Army Good Conduct Medal (2nd Award); Army Commendation Medal 2nd Oak Leaf Cluster; Noncommissioned Officer Professional Development Ribbon; Army Achievement Medal; Expert Marksmanship Qualification Badge with Rifle Bar; and Driver and Mechanic Badge with Driver-M Bar. Item 18 (Remarks) contains no entry indicating that she served in SWA. 4. On 14 March 2005, a Correction to DD Form 214 (DD Form 215) was completed on the applicant. This document amended Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) by adding the SWA Service Medal with 2 bronze service stars and Kuwait Liberation Medal-Saudi Arabia and Kuwait, and added the entry "SERVICE IN SOUTHWEST ASIA FROM 910213-910515" to Item 18 (Remarks)." 5. In connection with the processing of this case, a member of the Board staff reviewed the Department of the Army (DA) Operation Desert Shield/Storm Personnel Roster .This search confirms the applicant served in SWA performing duties in MOS 52D during the period 13 February 1991 through 15 May 1991. 6. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It also provides instruction for the preparation and distribution of the DD Form 214. Chapter 2 provides item-item preparation instructions for the DD Form 214. The instructions for Item 12f state that all overseas service completed during the period covered by the DD Form 214 will be entered. The instructions for Item 18 state, in pertinent part, that the entry “SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be included in Item 18 to document the overseas deployment of an active duty Soldier. DISCUSSION AND CONCLUSIONS: 1. The applicant's contention that the Item 18 entry regarding her SWA service, which was added by the DD Form 215 issued to her on 14 March 2005, was incorrect was carefully considered. However, there is insufficient evidence to support this claim. 2. The evidence of record confirms the applicant was deployed to SWA in support of Operation Desert Shield/Desert Storm, and that she served there from 13 February 1991 through 15 May 1991, as evidenced by the DA Operation Desert Shield/Storm Personnel Roster, which is the official source of SWA service dates. Therefore, absent evidence to the contrary, there is an insufficent evidentiary basis to support granting the requested relief.
