Exhibit B - Military Personnel Records (including advisory opinion, if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that his dates of service in Kuwait and Iraq be shown in item 18 (Remarks) on his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states that he was in Kuwait and Iraq. 3. The applicant provides deployment orders; a flight manifest; a duty roster; a “Spur” Certificate; and a copy of his DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant served in Operation Enduring Freedom and Operation Iraqi Freedom. 2. The applicant enlisted on 15 March 1996, served as a radio repairer, and was honorably discharged on 7 October 2004. 3. Item 18 (Remarks) on the applicant’s DD Form 214 does not show any deployments. His DD Form 214 shows the National Defense Service Medal, the Army Service Ribbon, and the Overseas Service Ribbon as authorized awards. 4. In support of his claim, the applicant provided deployment orders, dated 11 February 2003, which show he was deployed to Kuwait in support of Operation Enduring Freedom. 5. The applicant also provided a duty roster which shows he was assigned to Iraq. His Noncommissioned Officer Reports covering the periods November 2002 to September 2003 and March 2003 to September 2004 show he was deployed in support of Operation Iraqi Freedom. 6. Records at the Defense Finance and Accounting Service show the applicant served in Kuwait and Iraq from 9 March 2003 to 2 September 2003. 7. Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214. The regulation states, in pertinent part, that the country and dates of deployment for an active duty Soldier deployed with his or her unit during their continuous period of active service will be shown in item 18 on the DD Form 214. DISCUSSION AND CONCLUSIONS: Evidence of record shows the applicant served in Kuwait and Iraq from 9 March 2003 to 2 September 2003. Therefore, item 18 on his DD Form 214 will be corrected to these dates of service.
