THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of item 11 (Primary Specialty), of her DD Form 214 (Certificate of Release or Discharge from Active Duty), issued on 9 December 2004, to show her correct primary military occupational specialty (PMOS) of "71G10" instead of "91G10," that item 12a (Date entered AD [Active Duty] This Period), be corrected to show the entry "1998 06 25" (25 June 1998) instead of the entry "1998 12 28" (28 December 1998), and that item 14 (Military Education) be corrected to show the entry "Patient Admin Specialist." 2. The applicant states, in effect, that items 11, 12a, and 14 of her DD Form 214 are incorrect. 3. The applicant provides a copy of her enlistment contract, a copy of her diploma, and copies of her DD Form 214, issued on 6 August 1999 and 9 December 2004, in support of her request. CONSIDERATION OF EVIDENCE: 1. The applicant's record shows she enlisted in the Regular Army on 25 June 1998, in the pay grade of E-3, for 4 years. The applicant successfully completed basic combat training at Fort Leonard Wood, Missouri, and advanced individual training at Fort Sam Houston, Texas. On completion of her advanced training, she was awarded the military occupational specialty (MOS), 91B, Medical Specialist. 2. The applicant served until she was honorably released from active duty on 6 August 1999, under the provisions of Army Regulation 635-200, chapter 8, for pregnancy. She was transferred to the United States Army Reserve (USAR) Control Group. 3. Item 12a, of her DD Form 214, issued on 6 August 1999, shows the entry "1998 06 25" (25 June 1998). 4. After a break in service, she reenlisted on 10 February 2000, in pay grade E-3, as a patient administration specialist, for training and assignment in MOS 71G. She was advanced to pay grade E-4 on 28 February 2001. She served until she was released from active duty on 9 February 2004. She was transferred to the USAR Control Group (Reinforcement). 5. Item 11, of her DD Form 214, issued on 9 February 2004, shows the entry "91G10 00 Behavioral Science SP (Specialist) -- 3 YRS (years) - 10 MOS (months);" item 12a, shows the entry "1998 12 28" (28 December 1998); item 12c (Net Active Service This Period), shows the entry "0005 01 12" (5 years, 1 month, and 12 days); and item 14, shows the entry "Behavioral Science Specialist CRS (course), 15 Weeks, 2000" which are all incorrect. 6. The applicant's DA Form 2-1 (Personnel Qualification Record-Part II) covering the period 10 February 2000 to 9 February 2004 is unavailable for review by the Board. 7. The applicant provided a copy of her diploma which shows that she successfully completed the Patient Administration Specialist Course from 22 February 2000 to 11 April 2000, at Fort Sam Houston, that was 7 weeks in length. This course was misidentified in item 14, of the applicant's DD Form 214. 8. A review of the applicant's OMPF (Official Military Personnel File), on IPERMS (Integrated Personnel Electronic Records Management), failed to reveal that the applicant attended the Behavioral Science course, for 15 weeks, in 2000, or served as a behavioral science specialist while serving on active duty during her second period of service. 9. Army Regulation 635-5 (Separation Documents) established the standardized policy for preparing and distributing the DD Form 214. The regulation provides, in pertinent part, that the PMOS and all additional military occupational specialties (MOS) served in, for a period of one year or more, during the Soldier’s continuous active military service, will be entered in item 11 (Primary Specialty). For each MOS, list the title with the years and months served. For time determination, 16 days or more count as a month and basic training (BT) and advanced individual training (AIT) are not counted. 10. The same regulation stated that item 12a (Date Entered AD this Period) will be completed to show the beginning date of the continuous period of AD for issuance of the present DD Form 214, for which a DD Form 214 was not previously issued. Item 12c will be completed to show the amount of service, computed by subtracting item 12a from 12b (Separation Date This Period), less lost time, if any. Item 14 (Military Education) will list formal, in service training courses of 40 hours or more successfully completed during the period of service covered by title, length in weeks, and month and year completed. DISCUSSION AND CONCLUSIONS: 1. The evidence of record shows that the applicant enlisted for training and assignment in MOS 71G, as a patient administration specialist. She was awarded this MOS as her primary MOS (PMOS) on 11 April 2000. The MOS 71G was converted to 91G effective 11 July 2001. She served as a patient administration specialist from 12 April 2000 to 9 February 2004, the date of her release from active duty, which equates to 3 years and 10 months. Therefore, she is entitled to correction of item 11, of her DD Form 214, to show the entry "91G10 00 Patient Administration SP – 3 YRS – 10 MOS" instead of the entry "91G10 00 Behavioral Science SP – 3 YRS – 10 MOS." 2. The evidence show the applicant reenlisted in the Regular Army on 10 February 2000; however, her DD Form 214, issued on 9 February 2004, shows her date of entry as "28 December 1998, which is incorrect. Therefore, item 12a, of her DD Form 214, issued on 9 February 2004, should be corrected to show the entry "2000 02 10" (10 February 2000) instead of the entry "1998 12 28" (28 December 1998). 3. By adjusting item 12a, of her DD Form 214, issued on 9 February 2004, to show the correct date of entry, as 10 February 2000, her net active service now equates to 4 years. Therefore, she is entitled to correction of item 12c (Net Active Service This Period), of her DD Form 214, issued on 9 February 2004, to show the entry " 0004 00 00" (4 years) instead of the entry "0005 01 12" (5 years, 1 month, and 12 days). 4. The evidence of record shows that she completed the Patient Administration Specialist Course, at Fort Sam Houston, on 11 April 2000, which was 7 weeks in length. Therefore, she is entitled to correction of item 14, of her DD Form 214, issued on 9 February 2004, to show the entry "Patient Administration Specialist CRS, 7 WEEKS, April 2000."
