THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his records to show his overseas service in Iraq. 2. The applicant states that he served 10 months in Iraq in support of Operation Iraqi Freedom. 3. The applicant provides a copy of his DD Form 214 (Certificate of Release or Discharge from Active Duty); a memorandum, dated 27 January 2004, addressed to the Veterans Administration; his Enlisted Record Brief; an email message; his Post-Deployment Health Assessment; a Certificate of Achievement; a psychiatric evaluation; and a Chronological Record of Medical Care. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 14 February 2001. At the completion of basic training and advanced individual training, he was awarded military occupational specialty 63S (heavy wheeled vehicle mechanic). 2. The applicant’s Post-Deployment Health Assessment shows he arrived in Southwest Asia on 4 March 2003. This document indicates the areas in which he was mainly deployed as Kuwait and Iraq. 3. On an unknown date, the applicant was awarded a Certificate of Achievement for service during Operations Enduring Freedom and Iraqi Freedom while assigned to the 181st Transportation Battalion. The period of service is not indicated on the certificate. 4. The applicant was honorably discharged on 19 March 2004 under the provisions of Army Regulation 635-200, paragraph 5-17 by reason of a physical condition, not a disability. He completed 3 years, 1 month, and 6 days of active military service. 5. His DD Form 214 does not reflect any foreign service in item 12f (Foreign Service). 6. Item 18 (Remarks) on his DD Form 214 does not show he served in Kuwait or Iraq. 7. Army Regulation 635-5 (Separation Documents) directs that foreign service performed during the period covered by the DD Form 214 will be entered in item 12f. 8. Army Regulation 635-5 states, in pertinent part, that the country and dates of deployment for an active duty Soldier deployed with his or her unit during their continuous period of active service will be shown in item 18. DISCUSSION AND CONCLUSIONS: 1. The applicant’s deployment to Iraq in support of Operation Iraqi Freedom is not, but should be, reflected in item 18 of his DD Form 214. 2. The applicant does not specifically indicate the dates he was in Iraq. However, he states he served 10 months in Iraq in support of Operation Iraqi Freedom. His Post-Deployment Health Assessment shows he arrived in-theater on 4 March 2003. Using 4 March 2003 as a starting date, 10 months later would have been 3 January 2004. In the absence of evidence to the contrary, item 18 of his DD Form 214 should be corrected to show he deployed to Kuwait/Iraq from 4 March 2003 through 3 January 2004. 3. It would be appropriate to amend item 12f of his DD Form 214 to reflect 10 months of foreign service.
