THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that the DA Form 1059 (Service School Academic Evaluation Report) for the Basic Noncommissioned Officer Course (BNCOC), dated 8 September 2007, which shows an “X” in item 11b (Performance Summary/Achieved Course Standards) be removed from his military records. 2. The applicant states, in effect, that upon completion of BNCOC he received two DA Forms 1059, dated 8 September 2007. He contends that the DA Form 1059 which shows an “X” in item 11b is the incorrect form. 3. The applicant provides two DA Forms 1059, dated 8 September 2007. CONSIDERATION OF EVIDENCE: 1. The applicant entered active duty on 18 April 2001. He is currently serving on active duty in the rank of staff sergeant. 2. The applicant provided two DA Forms 1059, dated 8 September 2007, which show he attended BNCOC at the United States Training Center (USATC) Noncommissioned Officer (NCO) Academy at Fort Eustis, Virginia during the period 22 June 2007 to 8 September 2007 and passed the course. One DA Form 1059 shows an “X” in item 11b and one DA Form 1059 shows an “X” in item 11a (Performance Summary/Exceeded Course Standards). 3. Officials at the USATC NCO Academy at Fort Eustis, Virginia confirmed that the DA Form 1059 which shows the applicant exceeded course standards is the correct form. 4. A review of the applicant’s Official Military Personnel File (OMPF) on the Personnel Electronic Records Management System revealed a copy of both of the DA Forms 1059, dated 8 September 2007. DISCUSSION AND CONCLUSIONS: Officials at the USATC NCO Academy at Fort Eustis, Virginia confirmed that the DA Form 1059 which shows the applicant exceeded course standards is the correct form. Therefore, it would be appropriate to remove the DA Form 1059, dated 8 September 2007, which shows an “X” in item 11b from his OMPF.
