THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that item 17 (Member Was Provided Complete Dental Examination and All Appropriate Dental Services and Treatment within 90 Days Prior to Separation) be changed from “Yes” to “No.” 2. The applicant states that block 17 on his DD Form 214 indicates he was provided all appropriate dental services within 90 days of separation. He had 137 days terminal leave and permissive temporary duty (PTDY) and dental problems arose prior to his actual retirement date and the military could not complete all the required dental work. He states that the Department of Veterans Affairs (VA) was going to complete the remaining portion of the dental care needed but they could not because block 17 on his DD Form 214 was checked “Yes.” He really needs this care [dental] completed and his DD Form 214 corrected to indicate he did not receive all appropriate dental services prior to his separation from the military. 3. The applicant provides four Standard Forms 603A (Chronological Record of Dental Care) and his DD Form 214. CONSIDERATION OF EVIDENCE: 1. After having had prior enlisted service, the applicant was appointed as a Reserve warrant officer on 15 March 1994 with a concurrent call to active duty. He was promoted to Chief Warrant Officer Four (CW4) on 1 January 2006. 2. On 15 January 2008, the applicant received dental treatment on three of his teeth. The dentist indicated the applicant needed follow-up treatment on one of his teeth. 3. The applicant received follow-up dental treatment on 17 January 2008 on one of his teeth. The dentist indicated that the applicant needed follow-up treatment on one of his teeth. 4. The applicant received dental treatment on 6 February 2008 for treatment on one of his teeth. His chief complaint, “I need a root canal on the second from the last tooth on the top right.” The treatment on this tooth was completed. 5. The applicant returned to the dental clinic on 11 February 2008 for treatment. He stated that the tooth that recently received the root canal was sore and hard to chew on. 6. The applicant was released from active duty and retired on 31 March 2008. 7. Item 17 on the applicant’s DD Form 214 shows the entry “Yes.” 8. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. It states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge. DISCUSSION AND CONCLUSIONS: 1. The evidence of record shows the applicant received dental treatment on 11 February 2008, six weeks prior to his retirement date. 2. It appears he needed additional dental care after his treatment on 11 February 2008. However, his DD Form 214 shows he was not provided all appropriate dental services and treatment prior to his retirement on 31 March 2008. 3.
