THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that her Certificate of Release or Discharge from Active Duty (DD Form 214) be amended to show that she was awarded the Southwest Asia Service Medal and the Kuwait Liberation Medal (Saudi Arabia). She also requests that any and all medals to which she was awarded be added to her DD Form 214. 2. The applicant states that due to the fact that she is a Reservist, she has been given the run around while trying to get these facts straight in her record. She states that if for some reason, no one can assist her or if she is referred to another office, she will have no other choice but to file an Inspector General Complaint and/or get the media involved. 3. The applicant provides in support of her application, a copy of her Certificate of Release or Discharge from Active Duty. CONSIDERATION OF EVIDENCE: 1. On 9 March 1983, the applicant enlisted in the United States Army Reserve (USAR) in Philadelphia, Pennsylvania, for 6 years, in the pay grade of E-1. She went on to successfully complete her training as an administrative specialist. 2. On 6 December 1990, the applicant was ordered to active duty in support of Operation Desert Shield/Storm. She returned to the Continental United States on 10 May 1991. 3. The applicant was honorably released from active duty (REFRAD) on 5 June 1991, at the expiration of her term of service. The DD Form 214 that she was furnished at the time of her REFRAD shows that she was awarded the Army Service Ribbon, the Army Lapel Button, the National Defense Service, the Marksman Marksmanship Qualification Badge (Rifle M-16), the Noncommissioned Officer Professional Development Ribbon, and the Army Reserve Component Achievement Medal. 4. The applicant's DD Form 214 also shows that she was ordered to active duty in support of Operation Desert Shield/Desert Storm in accordance with Title 10 United States Code 763B, and that she had service in Southwest Asia from 20 December 1990 through 10 May 1991. 5. In a memorandum that appears to be dated 26 March 2007, the applicant was awarded the Armed Forces Reserve Medal with "X" Device, by the Commander, 1st Battalion, 417th Regiment, 1st Brigade (BCT), 98th Division (IT), in recognition of good standing service in the Selected Reserve, for the period 9 December 1995 through 8 December 2005. 6. Army Regulation 600-8-22 (Military Awards) provides that service in the Persian Gulf War is to be recognized by award of the Southwest Asia Service Medal to Army members who participated in Operations Desert Shield/Desert Storm in the designated area on or after 2 August 1990. A bronze service star is authorized for the Defense of Saudi Arabia (2 August 1990 to 16 January 1991) and the Liberation and Defense of Kuwait (17 January to 11 April 1991), and the Cease-Fire Campaign (12 April 1991 to 30 November 1995). 7. The Kuwait Liberation Medal awarded by the Kingdom of Saudi Arabia (KLM-SA) was approved on 3 January 1992 and is awarded to members of the Armed Forces of the United States who participated in the Persian Gulf War between 17 January 1991 and 28 February 1991. 8. The Kuwait Liberation Medal awarded by the Government of Kuwait (KLM-K) was approved on 9 November 1995 and is awarded to members of the Armed Forces of the United States who participated in the Persian Gulf War between 2 August 1990 and 31 August 1993. 9. Army Regulation 600-8-22 provides for award of the Armed Forces Reserve Medal. It is awarded for honorable and satisfactory service as a member of one or more of the Reserve Components of the Armed Forces of the United States for a period of 10 years. The conditions for award of the Armed Forces Reserve Medal include requirements for such service to be completed within 12 consecutive years and service in a regular component of the Armed Forces, tenure in elected state office, tenure as a member of a legislative body of the United States or a state, and service as a judge of a court of record of the United States, a state or territory or District of Columbia is excluded from credit to ward this award but does not constitute a break in service. DISCUSSION AND CONCLUSIONS: 1. It appears that errors were made during the preparation of the applicant's DD Form 214 which resulted in the omission of the Southwest Asia Service Medal with three Bronze Service Stars, the Kuwait Liberation Medal (Saudi Arabia), and the Kuwait Liberation Medal (Kuwait). In accordance with the applicable regulations, the applicant was entitled to these awards prior to her REFRAD on 5 June 1991. Therefore, her DD Form 214 should be amended to include these awards. 2. The applicant's records show that she was awarded the Armed Forces Reserve Medal with "X" Device on 26 March 2007, for the period 9 December 1995 through 8 December 2005. Therefore, this award should also be included on her DD Form 214. 3. In view of the foregoing, it would now be appropriate to correct the applicant's records as recommended below.
