THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that the dates of his service in Iraq be corrected on his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states, in effect, that his dates of service in Iraq as shown on his DD Form 214 are incorrect and contain a typographical error. 3. The applicant indicates that he is submitting copies of 2 orders; however, these orders were not associated with his application and were not available for review. CONSIDERATION OF EVIDENCE: 1. The applicant served in the Regular Army from 28 June 2004 through 29 January 2007. He is currently serving in the Pennsylvania Army National Guard. 2. The applicant's DA Form 2-1 (Personnel Qualification Record) shows that he served in Iraq from 10 October 2004 through 7 November 2005. 3. The applicant's DD Form 214 shows that he served in Iraq from "10 October 2005" through 7 November 2005. 4. Army Regulation 635-5 (Separation Documents) prescribes policies and procedures regarding separation documents (DD Form 214/5). Item 18(Remarks) lists the name of the country to which deployed and the inclusive dates of service in the country. Each entry is verified by the individual's service record. DISCUSSION AND CONCLUSIONS: 1. The applicant's dates of service in Iraq were from 10 October 2004 through 7 November 2005 as verified by his DA Form 20. 2. The applicant's DD Form 214 contains a typographic error by showing his dates of service in Iraq as "10 October 2005" through 7 November 2005. Therefore, he is entitled to have his record corrected as shown below.
