THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his records to show award of the Good Conduct Medal (1st Award), the Army Commendation Medal, the Army Reserve Components Achievement Medal (1st Oak Leaf Cluster), and completion of the Prescribed Load List/The Army Maintenance Management System (PLL/TAMMS) Clerk Course. 2. The applicant states, in effect, that the awards and the training are not shown on his records. 3. The applicant provides the following additional documentary evidence in support of his request: a. DD Form 214 (Certificate of Release or Discharge from Active Duty), dated 17 January 2007. b. DA Form 87 (Certificate of Training), dated 25 July 2003. c. Certificate, dated 27 October 2006, showing award of the Army Commendation Medal. d. Permanent Order 07-247-061, dated 4 September 2007, showing award of the Good Conduct Medal (1st Award). e. Memorandum, dated 11 August 2005, showing award of the Army Reserve Components Achievement Medal (1st Oak Leaf Cluster). CONSIDERATION OF EVIDENCE: 1. The applicant's records show he enlisted in U.S. Army Reserve (USAR) on 27 July 2001 and was trained in military occupational specialty (MOS) 63B (Light Vehicle Mechanic). His records also show he executed a 6-year reenlistment in the USAR on 17 November 2006. The highest rank he attained during his military service was sergeant (SGT)/E-5. 2. On 19 September 2005, the applicant was ordered to active duty in support of Operation Iraqi Freedom and subsequently served in Kuwait/Iraq from 15 December 2005 to 15 December 2006. He was honorably released from active duty to the control of his USAR unit on 17 January 2007. 3. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons awarded or Authorized) of the applicant's DD Form 214 shows he was awarded the Army Achievement Medal (2nd Award), the Army Reserve Components Achievement Medal, the National Defense Service Medal, the Global War on Terrorism Service and Expeditionary Medals, the Army Service Ribbon, the Overseas Service Ribbon, the Armed Forces Reserve Medal with "M" Device, and the Driver and Mechanic Badge with Mechanic Clasp. 4. Item 13 does not show award of the Good Conduct Medal (1st Award), the Army Commendation Medal, or the Army Reserve Components Achievement Medal (1st Oak Leaf Cluster). 5. Item 14 (Military Education) of the applicant's DD Form 214 show the entry Combat Lifesavers Course. Item 14 does not show the PLL/TAMMS Clerk Course. 6. Headquarters, 1st Personnel Commend, Permanent Orders 205-043, dated 28 October 2006, show the applicant was awarded the Army Commendation Medal for meritorious service during the period 19 September 2005 to 27 October 2006. 7. Headquarters, 478th Personnel Service Battalion, Salt Lake City, Utah, Permanent Order 07-247-061, dated 4 September 2007, shows the applicant was awarded the Good Conduct Medal (1st Award) for exemplary behavior, efficiency, and fidelity in active Federal military service from 10 February 2003 to 23 July 2004. 8. Headquarters, 478th Personnel Service Battalion, Fort Missoula, Montana, Memorandum, dated 11 August 2005, shows the applicant was awarded the Army Reserve Components Achievement Medal for three years of continuous service, for the period 27 July 2001 to 27 July 2004. This memorandum erroneously shows the entry "1 OLC," indicating that this is the applicant's second award of the Army Reserve Components Achievement Medal. 9. The applicant submitted a copy of his Certificate of Training that shows he completed the PLL/TAMMS Clerk Course at Mountain Post Training and Education Center, Fort Carson, Colorado, from 14 July 2003 to 25 July 2003. 10. Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, that the Army Reserve Components Achievement Medal is authorized for award to Army personnel including Active Guard Reserve officers in the rank of colonel and below for exemplary behavior, efficiency, and fidelity while serving as a member of an Army National Guard or Army Reserve troop program unit or as an individual mobilization augmentee. Since 3 March 1972 the medal has been authorized on completion of 4 years service with a Reserve Component unit. Individual must have completed 4 years of qualifying service on or after 3 March 1972 and before the effective date of this regulation (28 March 1995). Beginning on the effective date of this regulation, the period of qualifying service for award of the Army Reserve Components Achievement Medal is reduced from 4 to 3 years. This change is not retroactive. Such years of qualifying service must have been consecutive. A period of more than 24 hours between Reserve enlistments or officer’s service will be considered a break in service. Credit toward earning the award must begin anew after a break in service. The bronze oak leaf cluster is awarded to denote the second and succeeding awards of a certain decorations, among which is the Army Reserve Components Achievement Medal. 11. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The purpose of the separation document is to provide the individual with documentary evidence of their military service. It is important that information entered on the form should be complete and accurate. 12. Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214. It states, in pertinent part, that the Soldier's military education in Item 14 is obtained from the Enlisted Record Brief (ERB). It shows the formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 and includes title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, training courses for combat skills are not listed. DISCUSSION AND CONCLUSIONS: 1. Permanent Orders awarded the applicant the Army Commendation Medal and the Good Conduct Medal (1st Award) which are not shown on his DD Form 214, dated 17 January 2007; therefore, he is entitled to correction of his records to show these awards. 2. The evidence of record shows that the applicant demonstrated exemplary behavior, efficiency, and fidelity while serving as a member of the USAR for a consecutive 3-year period of USAR service without a break in service. Accordingly, he was awarded the Army Reserve Components Achievement Medal for the period 27 July 2001 to 27 Jul 2004; this award is shown on his DD Form 214, dated 17 January 2007. However, he could not have completed a second 3-year qualifying period of USAR service until 27 July 2007, well after the time he was separated from active duty. Therefore, no corrective action is required. 3. With respect to the applicant's PLL/TAMMS training, the evidence of record shows that this training was not completed during the period of service covered by the DD Form 214; therefore, no corrective action is required. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing award of the Army Commendation Medal and the Good Conduct Medal (1st Award). 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to the Army Reserve Components Achievement Medal and the PLL/TAMMS Clerk Course.
