THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that his records be corrected by adding the awards of the Bronze Star Medal, Meritorious Service Medal, Combat Action Badge, and Meritorious Unit Commendation to his separation document (DD Form 214) and military file. 2. The applicant states that he was awarded the Bronze Star Medal for his service while deployed to Iraq during Operation Iraqi Freedom II. However, the award was erroneously omitted from his files. As for the awards of the Meritorious Service Medal, Combat Action Badge, and Meritorious Unit Commendation, he received the awards after his retirement from the U.S. Army Reserve. 3. The applicant provides a copy of his DD Form 214; personal statement; Certificate of Award for the Bronze Star Medal; DA Form 638 (Recommendation for Award) with Permanent Orders 261-14, dated 17 September 2004; Certificate of Award for the Meritorious Unit Commendation with Permanent Orders   113-005, dated 23 April 2005; Permanent Orders C336-16, dated 2 December 2005 for the award of the Combat Action Badge; Certificate of Award for the Meritorious Service Medal; and DA Form 638 with Permanent Orders   C285-04, dated 12 October 2006. CONSIDERATION OF EVIDENCE: 1. The applicant was promoted as a Reserve Commissioned Officer of the Army on 12 May 1997 to the grade of major. 2. He was ordered to active duty in support of Operation Iraqi Freedom on   11 December 2003. He served in Iraq with the 415th Civil Affairs Battalion, 1st Infantry Division from 6 February to 18 October 2004. On 25 November 2004, he was released from active duty by reason of completion of required active service and transferred to his home station at Kalamazoo, Michigan. 3. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) does not show the awards of the Bronze Star Medal, Meritorious Service Medal, Combat Action Badge, and Meritorious Unit Commendation. 4. The Certificate of Award and DA Form 638 with Permanent Orders   261-14 dated 15 September 2004 issued by Headquarters, 1st Infantry Division shows that the applicant was awarded the Bronze Star Medal for exceptionally meritorious service while serving as a Civil Affairs Team Chief during Operation Iraqi Freedom from 1 December 2003 to 30 November 2004. 5. Certificate of Award for the Meritorious Unit Commendation with Permanent Orders 113-005, dated 23 April 2005, issued by the Multi-National Force-Iraq shows that the applicant was awarded the Meritorious Unit Commendation for extraordinary meritorious service while providing the 1st Infantry Division (Mechanized) with civil military operations and civil administration support during Operation Iraqi Freedom from 7 February 2004 to 4 October 2004. 6. The Certificate of Award and DA Form 638 with Permanent Orders C285-04, dated 12 October 2006, issued by Headquarters, United States Army Civil Affairs and Psychological Operations Command (Airborne), Fort Bragg shows that the applicant was awarded the Meritorious Service Medal for exceptionally meritorious service while serving as the Operations Area Damage Control Officer, Plans Officer S3, and Battalion Signal Officer during Operation Iraqi Freedom from 1 October 1995 to 29 September 2006. 7. Permanent Orders C336-16, dated 2 December 2005, issued by the same headquarters shows that the applicant was awarded the Combat Action Badge for actively engaging or being engaged by the enemy on 22 July 2004. DISCUSSION AND CONCLUSIONS: 1. The applicant contends that his record should be corrected by adding the awards of the Bronze Star Medal, Meritorious Service Medal, Combat Action Badge, and Meritorious Unit Commendation. 2. Evidence shows that the applicant was awarded the Bronze Star Medal, Meritorious Service Medal, and the Combat Action Badge. Evidence also shows that the applicant’s unit was awarded the Meritorious Unit Commendation during his tenure of assignment. Therefore, he is entitled to correction of his records to show these awards.
