Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty), with the period ending 3 July 2008, to show award of the Combat Infantryman Badge, the Army Commendation Medal, and the Army Good Conduct Medal. In addition, he further requests his DD Form 214 be corrected to show he completed two Army training courses. 2. The applicant states, in effect, that he earned the above awards. He further states at the time his DD Form 214 was prepared he did not have the actual orders and certificate available to have them added to his DD Form 214. 3. The applicant provides his DD Form 214; two Army certificates of training; two Army Commendation Medal certificates, with dates 15 December 2005 and 11 April 2008; Headquarters, Multi-National Corps Iraq Permanent Orders 346-214, dated 12 December 2005; and Headquarters, 1st Battalion, 504th Parachute Infantry Regiment Permanent Order Number 155-01, dated 3 June 2008. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 25 January 2005 and successfully completed basic training and advanced individual training. He was awarded military occupational specialty 11C (Indirect Fire Infantryman). 2. On 3 October 2005, the applicant was deployed to Iraq. 3. Headquarters, Multi-National Corps Iraq Permanent Orders 346-214, dated 12 December 2005, show that the applicant was awarded the Combat Infantryman Badge. 4. Headquarters, 43rd Military Police Brigade Permanent Order Number 349-227, dated 15 December 2005, shows that the applicant was awarded the Army Commendation Medal. 5. On 14 February 2006, the applicant departed from Iraq after being credited with participating in two campaigns. 6. An Army certificate of training shows the applicant successfully completed the Force XXI Battle Command Brigade and Below (FBCB2) Operational Network Course during October 2006 at Fort Bragg, North Carolina (course length 32 hours). 7. An Army certificate of training shows the applicant successfully completed the Modern Army Combatives Level I Course during the period 11 December 2006 through 15 December 2006 at Fort Bragg, North Carolina (course length 40 hours). 8. Headquarters, 2nd Brigade Combat Team Permanent Orders Number 331-85, dated 11 April 2008, shows the applicant was awarded the Army Commendation Medal. 9. Headquarters, 1st Battalion, 504th Parachute Infantry Regiment Permanent Orders 155-01, dated 3 June 2008, shows the applicant was awarded the Army Good Conduct Medal for the period 25 January 2005 through 24 January 2008. 10. On 3 July 2008, the applicant was honorably released from active duty after completing 3 years, 5 months, and 9 days of creditable active service. 11. The applicant's DD Form 214 shows he was awarded the Army Commendation Medal, the Army Achievement Medal, the National Defense Service Medal, the Global War on Terrorism Service Medal, the "Iraq Campaign Medal Campaign Star," the Army Service Ribbon, the Overseas Service Ribbon, and the Parachutist Badge. 12. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states for item 14 (Military Education), list in-service training course; title, number of weeks, year successfully completed during this period of service; e.g., medical, dental, electronics, supply, administration, personnel, or heavy equipment operations. This information is to assist the member after separation in job placement and counseling; therefore, training courses for combat skills will not be listed. 13. Army Regulation 600-8-22 states that individuals authorized the Iraq Campaign Medal must have served in direct support of Operation Iraqi Freedom. The area of eligibility encompasses all land area of the country of Iraq, and the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of Operation Iraqi Freedom. A bronze service star is authorized for the following campaigns: Liberation of Iraq 19 March 2003 – 1 May 2003, Transition of Iraq 2 May 2003 – 28 June 2004, Iraqi Governance 29 June 2004 – 15 December 2005, and National Resolution 16 December 2005 – Date to be determined. DISCUSSION AND CONCLUSIONS: 1. Evidence of record shows that the applicant was awarded the Combat Infantryman Badge, the Army Commendation Medal 1st Oak Leaf Cluster and the Army Good Conduct Medal. These awards should be added to his DD Form 214. 2. The applicant met the eligibility criteria for award of two bronze service stars to be worn on his already-awarded Iraq Campaign Medal. This award should be added to his DD Form 214. 3. The applicant's FBCB2 Operational Network Course was less than one week in length, making it not eligible for entry on his DD Form 214. 4. The applicant's Modern Army Combatives Level I Course was a training course for combat skills, making it not eligible for entry on his DD Form 214. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected: a. by deleting the entry "Iraq Campaign Medal Campaign Star" in item 13 of his DD Form 214; and b. by amending his DD Form 214 in item 13 to add the Iraq Campaign Medal with two bronze service stars, the Combat Infantryman Badge, the Army Commendation Medal 1st Oak Leaf Cluster, and the Army Good Conduct Medal. 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
