THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Achievement Medal and two bronze service stars to be affixed to his already-awarded Iraq Campaign Medal. 2. The applicant states that his Army Achievement Medal was approved after his separation and that the campaign stars for his Iraq Campaign Medal were enacted after his separation. 3. The applicant provides a copy of his DD Form 214, dated 1 March 2007, and a copy of a certificate, dated 1 March 2007, showing award of the Army Achievement Medal in support of his request. CONSIDERATION OF EVIDENCE: 1. The applicant's records show he was appointed as a U.S. Army Reserve commissioned officer in the rank of second lieutenant and executed an oath of office on 21 July 2002. He was ordered to active duty on 27 November 2002 and subsequently completed the Engineer Officers Basic Course. He was promoted to first lieutenant on 27 May 2004 and captain on 11 June 2006. He was honorably discharged on 1 March 2007. 2. The applicant’s records also show he served in Iraq from 31 October 2005 to 30 October 2006. He was assigned to the 14th Engineer Battalion. 3. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 shows he was awarded the Bronze Star Medal, the Army Commendation Medal (2nd Award), the National Defense Service Medal, the Global War on Terrorism Service Medal, the Korea Defense Service Medal, the Iraq Campaign Medal, the Army Service Ribbon, and the Overseas Service Ribbon. Item 13 does not show award of the Army Achievement Medal and/or two bronze service stars to be affixed to the Iraq Campaign Medal. 4. Headquarters, 14th Engineer Battalion, Fort Lewis, Washington, Permanent Orders Number 017-001, dated 1 March 2007, show the applicant was awarded the Army Achievement Medal for meritorious service from 24 July 2004 to 1 March 2007. 5. Military Personnel (MILPER) Message 08-123, dated 30 April 2008, provided guidance on the criteria, applicability, and standards for awarding and the wear of service stars on the Iraq Campaign Medal. This message states, in pertinent part, that a bronze service star will be awarded for wear on the Iraq Campaign Medal for participation in each of the following four campaigns: Liberation of Iraq, 19 March 2003 to 1 May 2003; Transition of Iraq, 19 March 2003 to 1 May 2003; Iraqi Governance, 29 June 2004 to 15 December 2005; and the National Resolution, 16 December 2005 to a date to be determined. DISCUSSION AND CONCLUSIONS: 1. Permanent Orders awarded the applicant the Army Achievement Medal for meritorious service from 24 July 2004 to 1 March 2007 which is not shown on his records; therefore, he is entitled to correction of his records to show this award. 2. The evidence of record shows that the applicant was awarded the Iraq Campaign Medal. Furthermore, the applicant participated in two campaigns while serving in Iraq and is therefore entitled to award of two bronze service stars to be affixed to the Iraq Campaign Medal.
