The evidence shows that the applicant is entitled to a correction to Item 12f. of his DD Form 214 to show he served in Iraq for 00 years, 02 months, and 01 days foreign service. The applicant is further entitled to have Item 18 corrected to show this same service in Iraq for the period 5 October 2004 through 5 December 2004. 3. The evidence shows the applicant was awarded the Army Achievement Medal while he served in Iraq. He is therefore entitled to a correction to his DD Form 214 to show he was awarded the Army Achievement Medal (3rd Award). 4. Based on the applicant's service in Iraq for the period 5 October 2004 through 5 December 2004 in support of Operation Iraqi Freedom, he is entitled to award of the Global War on Terrorism Service Medal and the Iraq Campaign Medal and to have these awards added to his DD Form 214. 5. The evidence shows the applicant was mobilized in support of Operation Iraqi Freedom. As a result of his mobilization, the applicant is authorized the Armed Forces Reserve Medal with "M" Device. The Armed Forces Reserve Medal with "M" Device is not shown on his DD Form 214. He is entitled to a correction of his DD Form 214 to add this award. 6. The evidence shows the applicant served a qualifying period of service in the Republic of Korea. He is therefore entitled to award of the Korea Defense Service Medal and to have this award added to his DD Form 214.
