THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that the date of entry on her DD Form 214 (Certificate of Release or Discharge from Active Duty) with a separation date of 30 September 2009 be changed to 1989 09 14 (14 September 1989) instead of 2006 07 01 (2006 July 01). 2. The applicant states the entry in item 12a (Date Entered AD (active duty) This Period) is incorrect. 3. The applicant provides a copy of her: * DD Form 214 * Enlisted Record Brief CONSIDERATION OF EVIDENCE: 1. The applicant's Official Military Personnel File (OMPF) that is in the integrated Personnel Electronic Records Management System (iPERMS) contains the following enlistment contracts: * DD Form 4/3 shows she enlisted on 14 September 1989 for a period of 4 years * DD Form 4/1 shows she reenlisted on 15 April 1993 for a period of 4 years * DD Form 4/1 shows she reenlisted on 27 August 1996 for a period of 4 years * DD Form 4/1 shows she reenlisted on 29 August 1999 for a period of 2 years * DD Form 4/1 shows she reenlisted on 23 July 2001 for an indefinite period 2. On 30 September 2009, the applicant was retired and transferred to the Retired List. Item 12a of her DD Form 214 contains the entry 2006 07 01. 3. Army Regulation 635-5 (Separation Documents), effective 1 October 1979, stated that effective 1 October 1979, DD Forms 214 would no longer be issued for enlisted members who were discharged for immediate reenlistments in the Regular Army and that all service would be continuous from the date the last DD Form 214 was issued. 4. Army Regulation 635-5 currently states the beginning date of the continuous period of AD for which the DD Form 214 is being issued be entered in Item 12a of the DD Form 214. DISCUSSION AND CONCLUSIONS: 1. The applicant contends the entry in item 12a of her DD Form 214 should be 14 September 1989. 2. Effective 1 October 1979 DD Forms 214 were no longer issued to Soldiers who were separated for the purpose of immediate reenlistment. 3. The enlistment contracts in the applicant's OMPF show she continuously served in an active duty status from 14 September 1989 until she retired on 30 September 2009. 4. Therefore, the entry in item 12a of her DD Form 214 should be corrected to show the entry 1989 09 14 instead of 2006 07 01.
