THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty), for the period ending on 10 May 2007, to show his foreign service in Afghanistan and all the awards he received pertaining to this service. 2. The applicant states he did not notice his first DD Form 214 for the period ending on 10 May 2007 omitted his service in Afghanistan from 2006 to 2007. He also states none of the awards he received while serving in Afghanistan are shown on his DD Form 214. 3. The applicant provides the following documents: * DD Form 214, dated 10 May 2007 * DD Form 214, dated 25 March 2010 * DA Form 638 (Recommendation for Award) showing award of the Army Commendation Medal * Orders awarding him the Combat Action Badge * Certificate awarding him the NATO Medal CONSIDERATION OF EVIDENCE: 1. The applicant's records show he enlisted in the Regular Army for a period of 3 years on 2 March 2004 and held military occupational specialty 52D (Power Generator Equipment Repairer). He was assigned to the 37th Engineer Battalion, Fort Bragg, NC. 2. Permanent Orders Number 010-026, issued by Headquarters, Combined Task Force Chamberlain, Afghanistan, on 27 January 2007, awarded him the Army Commendation Medal for meritorious service in support of Operation Enduring Freedom (OEF) in Afghanistan from 11 March 2006 to 10 March 2007. 3. Permanent Orders 362-042, issued by Headquarters, Combined/Joint Task Force 76, Afghanistan, on 28 December 2006, awarded him the Combat Action Badge for being engaged by the enemy on 20 July 2006 in Afghanistan. 4. His records contain a certificate showing he was awarded the NATO Medal for service with NATO in Afghanistan from 13 March 2006 to 8 March 2007. 5. He was honorably released from active duty on 10 May 2007 and transferred to the Mississippi Army National Guard (MSARNG). The DD Form 214 he was issued for this period of service shows he completed 3 years and 2 months of creditable active military service. This form further shows the following entries: * Item 12f (Foreign Service) shows the entry "0000 00 00" * Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) shows he was awarded the National Defense Service Medal, Global War on Terrorism Service Medal, and Army Service Ribbon * Item 18 (Remarks) does not list service in a designated hostile fire pay/imminent danger pay (HFP/IDP) area 6. He enlisted in the MSARNG on 11 May 2007 and he was subsequently assigned to the 106th Brigade Support Battalion, MSARNG. 7. He was ordered to active duty on 1 May 2009 and subsequently served in Iraq in support of Operation Iraqi Freedom from 9 July 2009 to 7 March 2010. He was honorably released from active duty to the control of his ARNG unit on 25 March 2010. The DD Form 214 he was issued for this period of service shows he completed 10 months and 25 days of creditable active military service. This form further also shows the following entries: * Item 12f shows the entry "0000 07 25" * Item 13 shows the Army Commendation Medal, Army Good Conduct Medal, National Defense Service Medal, Global War on Terrorism Service Medal, Iraq Campaign Medal with a bronze service star, Overseas Service Ribbon, and Armed Forces Reserve Medal with "M" Device * Item 18 lists his service in Iraq from 9 July 2009 to 7 March 2010 8. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The purpose of the separation document is to provide the individual with documentary evidence of their military service. It is important that information entered on the form should be complete and accurate. Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214. It states item 12 shows the Record of Service. Extreme care is used when completing this block since post-service benefits, final pay, retirement credit, and so forth are based on this information. Item 12f is obtained from the Enlisted/Officer Record Brief and shows the total amount of foreign service completed during the period covered in by the DD Form 214. Additionally, for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "Service in (Name of County Deployed) from (inclusive dates for example, YYYYMMDD – YYYMMDD)” is entered in item 18. 9. Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who have served in direct support of OEF. The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 24 October 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved designated Afghanistan campaigns are: * Liberation of Afghanistan (11 September 2001-30 November 2001) * Consolidation I (1 December 2001-30 September 2006) * Consolidation II (1 October 2006-date to be determined DISCUSSION AND CONCLUSIONS: 1. With respect to his service in Afghanistan, the evidence of record shows he served in Afghanistan from 11 March 2006 to 10 March 2007 as confirmed by his award of the Army Commendation Medal and the NATO Medal. Therefore, he is entitled to correction of his DD Form 214 ending on 10 May 2007 to show completion of 12 months of foreign service in an HFP/IDP area, Afghanistan. 2. This period of service also entitles him to award of the Afghanistan Campaign Medal. Furthermore, as a result of his campaign participation, he is also authorized two bronze service stars to be affixed to the Afghanistan Campaign Medal; therefore, he is entitled to correction of his DD Forms 214 to show these awards. 3. The evidence of record shows he was awarded the NATO Medal and the Combat Action Badge which are not shown on either of his DD Forms 214; therefore, he is entitled to correction of both of his DD Forms 214 to show these awards.
