THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show: * service in Iraq * decorations and awards, including the Southwest Asia Service Medal 2. The applicant states his DD Form 214 does not reflect is Iraq service or his awards for Operation Iraqi Freedom (OIF). 3. The applicant provides: * Certificate of Achievement, 141st Signal Battalion, 1st Armored Division, Coalition Joint Task Force (CJTF)-7, Baghdad, Iraq, dated 7 April 2004 * U.S. Army Human Resource Systems Soldier Deployment History Outprocessing Report, dated 5 March 2010 * County of Humboldt Veterans Service Office letter, dated 15 April 2010 CONSIDERATION OF EVIDENCE: 1. The applicant served in the Regular Army from 11 February 2003 through 16 March 2010. 2. The applicant's DD Form 214 shows: * in tem 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) * Army Achievement Medal * Army Good Conduct Medal (2nd Award) * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon * in item 18 (Remarks) * Immediate Reenlistments This Period – 20030211-20060316 * Subject to Active Duty Recall, Muster Duty, and/or Annual Screening * Block 6, Period of Delayed Entry Program: 20021115-20030210 * Member has Completed First Full Term of Service 3. The applicant submitted a Soldier Deployment History Outprocessing Report which shows he served 267 days of war service from 11 October 2003 through 4 July 2004. 4. The Defense Finance and Accounting Service records show the applicant received hostile fire/imminent danger pay and combat zone tax exclusion for Kuwait from 11 October 2003 through 16 July 2004. 5. The CJFT Certificate of Appreciation shows the applicant's service was in Iraq. 6. Army Regulation 600-8-22 (Military Awards) states: a. The Southwest Asia Service Medal is awarded to members of the Armed Forces of the United States who participated in Operations Desert Shield/ Desert Storm in the designated area on or after 2 August 1990 to 30 November 1995. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved designated campaigns are: * Defense of Saudi Arabia (2 August 1990 to 16 January 1991) * Liberation and Defense of Kuwait (17 January 1991 to 11 April 1991) * Cease-Fire Campaign (12 April 1991 to 30 November 1995) b. The Iraq Campaign Medal is awarded to members who served in direct support of OIF. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of OIF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are: * Liberation of Iraq (19 March 2003-1 May 2003) * Transition of Iraq (2 May 2003-28 June 2004) * Iraqi Governance (29 June 2004-15 December 2005) * National Resolution (16 December 2005-9 January 2007) * Iraqi Surge (10 January 2007-31 December 2008) * Iraqi Sovereignty (1 January 2009-date to be determined) 7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, or release from active duty service or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214 and provides: a. In item 13, list awards and decorations for all periods of service in priority sequence. Do not use abbreviations. b. For an active duty Soldier deployed with his or her unit during their continuous period of active service, enter in item 18: "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM YYYYMMDD-YYYYMMDD." DISCUSSION AND CONCLUSIONS: 1. The applicant requests recognition for his Iraq service on his DD Form 214. 2. Army Regulation 635-5 mandates foreign service recognition for unit deployments to Southwest Asia. The applicant served in Iraq from 20031011-20040704. This should be annotated in item 18. 3. Based on his Iraq service, the applicant is entitled to the Iraq Campaign Medal with two bronze service stars representing the Transition of Iraq Campaign and the Iraqi Governance Campaign. This award should be added to item 13 of his DD Form 214. 4. The applicant is not entitled to the Southwest Asia Service Medal. This medal was issued for Soldiers who participated in the First Persian Gulf War (Operations Desert Shield/Desert Storm). The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: * adding the Iraq Campaign Medal with two bronze service stars to item 13 of his DD Form 214 * adding "SERVICE IN IRAQ FROM 20031011-20040704" to item 18 of his DD Form 214 2. The Board further determined the evidence presented was insufficient to warrant a portion of the requested relief.
