THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Presidential Service Badge, Number 23054. 2. He states the Presidential Service Badge was not added to his DD Form 214 because of an omission of data. 3. He provides a copy of a letter from the Deputy Assistant to the President and Director, White House Military Office, dated 1 March 2008. CONSIDERATION OF EVIDENCE: 1. After completing prior enlisted service, the applicant was appointed as a Reserve warrant officer on 28 March 1997 with a concurrent call to active duty. 2. He provided a letter from the Deputy Assistant to the President and Director, White House Military Office, dated 1 March 2008, which shows he was awarded the Presidential Service Badge and Certificate in recognition of his service to the President of the United States under the provisions of Executive Order 12793, dated 20 March 1992. The letter also shows he was issued the certificate and the badge. 3. He was honorably retired by reason of sufficient service for retirement on 30 June 2010. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 does not show the Presidential Service Badge as an authorized badge. 4. Army Regulation 600-8-22 (Military Awards) states the Presidential Service Badge and the Presidential Service Certificate were established by Executive Order 11174, 1 September 1964 as amended by Executive Orders 11407, 23 April 1968; Executive Order 11520, 25 March 1970; and Executive Order 12793, 20 March 1992, as amended by Executive Order 13286, 28 February 2003. This award replaced the White House Service Badge and Certificate established by Executive Order 10879, 1 June 1960. DISCUSSION AND CONCLUSIONS: The applicant has provided sufficient evidence which shows he was awarded the Presidential Service Badge by Executive Order 12793, dated 20 March 1992. Therefore, he is entitled to correction of his DD Form 214 to show this badge.
