Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of item 18 (Remarks) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her deployment to Kuwait and Afghanistan. 2. The applicant states she was on orders and deployed from January to February 2007. 3. The applicant provides her DD Form 214 in support of her application. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 14 January 2004 and trained as a human resources specialist. On 21 December 2007, she was honorably discharged for parenthood. 2. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of her DD Form 214 shows, among other awards, the Global War on Terrorism Service Medal as an authorized award. Item 18 of her DD Form 214 does not show any deployments. 3. Records at the Defense Finance and Accounting Service (DFAS) show she received hostile fire/imminent danger pay during the period 26 January 2007 to 11 February 2007 for service in Kuwait. 4. No evidence shows she served in Afghanistan. 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. The regulation states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)" will be entered in item 18. DISCUSSION AND CONCLUSIONS: 1. The DFAS information is accepted as sufficient evidence on which to amend item 18 of her DD Form 214 to show she served in Kuwait. Therefore, item 18 of her DD Form 214 should be corrected to show she served in Kuwait from 26 January 2007 to 11 February 2007. 2. The applicant also contends she deployed to Afghanistan in January/February 2007. However, there is no evidence and she provided no evidence to support this contention. Therefore, there is insufficient evidence on which to amend item 18 of her DD Form 214 to show deployment to Afghanistan. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the entry "SERVICE IN KUWAIT FROM 20070126-20070211" to item 18 of her DD Form 214. 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief.
