Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her foreign service in Iraq. 2. She states a mistake was made while completing her DD Form 214 at Fort Sam Houston, TX. 3. She provided: * a Soldier Deployment History Outprocessing Report * her DD Form 214 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army (RA) on 3 January 2000 for a period of 4 yours. She served in military occupational specialty 92A (Automated Logistical Specialist). On 20 February 2002, she reenlisted for a period of 4 years. On 28 March 2005, she again reenlisted for a period of 6 years. On 20 March 2008, she was honorably discharged to accept a commission in the Army. 2. On 21 March 2008, a DA Form 71 (Oath of Office - Military Personnel) shows the applicant executed an oath of office as a commissioned officer in the RA as a second lieutenant (2LT)/O-1 in the Army Nurse Corps. She is currently serving in the RA as a first lieutenant. 3. She provided a Soldier Deployment History Outprocessing Report that shows she served in a named campaign for the purpose of war from 16 November 2004 through 5 April 2005. 4. Her DD Form 214 for the period ending 20 March 2008 shows in: * Item 12f (Foreign Service) the entry "0000 00 00" indicating no foreign service during this period of service * Item 18 (Remarks) does not reflect any deployed service 5. During the processing of this case, the Defense Finance and Accounting Service (DFAS) was asked to verify the applicant's dates of foreign service in Iraq. In response, a representative from DFAS stated that per the applicant's Master Military Pay Account (MMPA), she received hostile fire/imminent danger pay (HF/IDP) and combat zone tax exclusion (CZTE) for Kuwait from 16 November 2004 through 5 April 2005, which equates to a total of 4 months and 21 days of foreign service. 6. Army Regulation 635-5 (Separation Documents) states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. It is important that information entered on the form should be complete and accurate. Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214. It states, in pertinent part, that for: a. Item 12f, enter the total amount of foreign service completed during their continuous period of active service; b. Item 18, for an active duty Soldier deployed with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)" will be entered. DISCUSSION AND CONCLUSIONS: Evidence of personnel and DFAS records confirm she served in Kuwait/Iraq and received HF/IDP and CTZE for from 16 November 2004 through 5 April 2005. Therefore, her DD Form 214 should be corrected to show she served in Kuwait/Iraq from 16 November 2004 through 5 April 2005 for a period of 4 months and 21 days.
