THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to reflect all of his service. 2. The applicant states, in effect, he was initially ordered to active duty at Fort Hood, Texas, on 20 April 2006 and served there for 2 years before being transferred to Fort Sam Houston, Texas, on 17 September 2007. He served there until he was released from active duty (REFRAD) on 19 February 2009. However, his DD Form 214 only reflects his service from 17 September 2007 instead of 20 April 2006. 3. The applicant provides copies of: * his DD Form 214 * his DD Form 214WS (DD Form 214 Worksheet) * his permanent change of station (PCS) orders to Fort Sam Houston, Texas CONSIDERATION OF EVIDENCE: 1. The applicant served in the U.S. Air Force from 30 November 1976 to 21 April 1982. He completed 5 years, 7 months, and 22 days of active service and 4 months and 18 days of inactive service. 2. On 26 September 2003, he was appointed as a U.S. Army Reserve second lieutenant in the Army Nurse Corps. 3. He was ordered to report for active duty at Fort Hood, Texas, on 20 April 2006 and on 17 September 2007 he was transferred to Fort Sam Houston, Texas. He was promoted to the rank of captain on 1 May 2008. 4. On 19 February 2009, he was honorably REFRAD due to the completion of his required service. His DD Form 214 issued at the time of his REFRAD shows he entered this period of active duty on 17 September 2007 and that he completed 1 year, 5 months, and 3 days of active service during this period. It reflects no prior active or inactive service. 5. The applicant was again ordered to active duty on 2 March 2009 and served until he was honorably REFRAD on 1 March 2010. His DD Form 214 shows that he had served 1 year of active duty and reflects no prior active or inactive service. 6. Army Regulation 635-5 serves as the authority for the preparation of the DD Form 214. It provides that the DD Form 214 will be prepared to reflect all active service performed during the period covered by the DD Form 214 and all prior active and inactive service. DISCUSSION AND CONCLUSIONS: 1. The applicant's contentions have been noted and were found to have merit. He entered active duty on 20 April 2006. However, his DD Form 214 issued on 19 February 2009 incorrectly reflects that he entered active duty on 17 September 2007. 2. Additionally, it reflects that he only served 1 year, 5 months, and 3 days of active service when he, in fact, completed 2 years and 10 months of active service. He also completed 5 years, 7 months, and 22 days of prior active service and 4 months and 18 days of prior inactive service. 3. The applicant's DD Form 214 issued on 1 March 2010 does not reflect that the applicant had served 8 years, 5 months and 22 days of prior active service and 4 months and 18 days of prior inactive service. 4. Accordingly, his DD Forms 214 dated 19 February 2009 and 1 March 2010 should be corrected to reflect all of his service.
