THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests the Bronze Star Medal (BSM) be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 27 February 2011.

2.  The applicant states approval of the award was subsequent to the issuance of his DD Form 214.

3.  The applicant provides:

* Award certificate for the BSM
* DA Form 638 (Recommendation for Award)
* DD Form 214 for the period ending 27 February 2011
* Discharge orders

CONSIDERATION OF EVIDENCE:

1.  Having prior service in the Army National Guard, he was ordered to active duty on 5 December 2009 in support of Operation Enduring Freedom.  He served as an infantry officer in Afghanistan from 3 March 2010 to 6 December 2010 and he was released from active duty on 27 February 2011.

2.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 for the period ending 27 February 2011 does not show the BSM as an authorized award. 

3.  His award certificate, dated 7 December 2010, shows he received the BSM for exceptionally meritorious service in Afghanistan from 6 March 2010 to 
19 November 2010 on Permanent Orders Number 341-036. 

DISCUSSION AND CONCLUSIONS:

Permanent orders show he received the BSM.  His DD Form 214 for the period ending 27 February 2011 should be corrected to show this medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the BSM to item 13 of his DD Form 214 for the period ending 27 February 2011.
