THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show: * her correct service * completion of postal training 2. The applicant states her service was not calculated properly due to the start date of her Reserve deployment order being reported using the wrong date. This error caused her total time in service to be miscalculated. 3. The applicant provides her DD Form 214 for the period ending 27 December 2005. CONSIDERATION OF EVIDENCE: 1. The applicant's records show she enlisted in the U.S. Army Reserve on 9 October 2003. 2. On 18 December 2003, the applicant entered active duty for training (ADT), completed the required training, and she was awarded military occupational specialty (MOS) 42L (Administrative Specialist). She was released from ADT to the control of her Reserve unit on 5 May 2004. She completed 5 months and 18 days of creditable active service during this period. 3. Her records also show she completed the Postal Operations Course from 29 March 2004 through 27 April 2004 (4 weeks or 30 days). She was also awarded primary MOS 42L1OF5 (the F5 is the additional skill identifier (ASI) for postal operations). This course is listed on her DD Form 214 for the period ending 5 May 2004. 4. She was ordered to active duty on 1 November 2004 in support of Operation Iraqi Freedom and subsequently served in Kuwait/Iraq from 1 December 2004 to 6 December 2005. 5. She was honorably released from active duty on 27 December 2005. She completed 1 year, 1 month, and 27 days of active service during this period. Her DD Form 214 for this period of service shows in: * Item 12a (Date Entered Active Duty (AD) This Period) - 2005 11 01 * Item 12b (Separation date This Period) - 2005 12 27 * Item 12c (Net Active Service This Period) - 0001 01 27 * Item 12d (Total Prior Active Service) - 000 05 18 * Item 14 (Military Education) - "None" 6. She was issued a DD Form 215 (Correction to DD Form 214) on 29 December 2011 that corrected her date of entry in item 12a to show "2004 11 01" instead of "2005 11 01." 7. She enlisted in the Regular Army on 8 May 2006 for 4 years and she also executed a 15-month extension on 6 January 2010. During this period, she served in Kuwait/Iraq from 21 October 2007 to 9 January 2009 and from 13 June 2010 to 29 May 2011. 8. She was honorably released from active duty on 7 August 2011. She completed 5 years and 3 months of active service during this period. Her DD Form 214 for this period of service shows in: * Item 12a - 2006 05 08 * Item 12b - 2011 08 07 * Item 12c - 0005 03 00 * Item 12d - 0000 07 15 9. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance on the preparation of the DD Form 214. It states that item 12 shows the record of service. Extreme care is used when completing this item since post-service benefits, final pay, retirement credit, and so forth are based on this information. Furthermore, it states for: * item 12a – enter the beginning date of the continuous period of active duty for issuance of this DD Form 214 * item 12b – enter the Soldier's transition date, less time lost * item 12c – enter the amount of active service this period, computed by subtracting item 12a from 12b, less time lost * item 12d – enter the total amount of prior active military service less time lost, if any, obtained from previously-issued DD Forms 214 * item 14 – enter the formal in-service training courses completed during the period covered by the DD Form 214; combat skills are not listed DISCUSSION AND CONCLUSIONS: 1. With respect to the DD Form 214 for the period 1 November 2004 through 6 December 2005: a. The applicant enlisted in the USAR on 9 October 2003. She subsequently completed a total of 5 months and 18 days of active service between 18 November 2003 and 5 May 2004. She completed the Postal Operations Course in April 2004, and this course is properly listed on her DD Form 214 for the period ending 5 May 2004. b. She served on active duty from 1 November 2004 through 6 December 2005, a period of 1 year, 1 month, and 27 days of active service. Her DD Form 214 inadvertently listed her date of entry as 1 November 2005. However, she was issued a DD Form 215 that corrected her date of entry. 2. With respect to the DD Form 214 for the period 8 May 2006 through 7 August 2011: a. She completed 5 years and 3 months of active service during this period, which is appropriately shown on her DD Form 214. b. She previously completed a total of 1 year, 7 months, and 15 days (5 months and 18 days of training plus 1 year, 1 month, and 27 days of mobilization). Her DD Form 214 inadvertently listed this period of prior active service as 7 months and 15 days. Therefore, she is entitled to correction of this DD Form 214 to show the correct period of prior active service. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from item 12d of her DD Form 214 for the period ending 7 August 2011 the entry "0000 07 15" and adding the entry "0001 07 15." 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
