Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her record to change her last name and address. 2. She states she desires her name be changed due to her recent marriage and that her address is now in Staunton, VA. 3. She provides a Marriage License and Certificate. CONSIDERATION OF EVIDENCE: 1. On 1 December 1967, the applicant accepted appointment as a Reserve commissioned officer of the Army. On 22 September 1968, she entered active duty and served until she was honorably relieved from active duty on 24 February 1971. On 12 October 1972, she was honorably discharged from the U.S. Army Reserve. Her record shows she used the last name C------ during this period of service. 2. On 26 August 1991, she entered active duty again. During this period of active duty service, she submitted a request to change her last name from P----- to W----r. The change was approved on 14 July 1994. 3. On 30 March 1995, she was retired for temporary disability. Her DD Form 214 (Certificate of Release or Discharge from Active Duty) shows the last name W----r and shows her mailing address after separation was to be an address in Williamsburg, VA. 4. On 22 February 2000, she was removed from the Temporary Disability Retired List and permanently retired. The orders effecting her permanent retirement show the last name W----r and show she resided at an address in Fort Defiance, VA. 5. She provides a Marriage License and Certificate showing she married J--- G----- W------l on 17 December 2011. The document shows the groom's usual residence is an address in Staunton, VA. 6. Army Regulation 635-5 prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It states the DD Form 214 is a synopsis of the Soldier’s most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge. DISCUSSION AND CONCLUSIONS: 1. For historical purposes, the Army has an interest in maintaining the integrity of its records. The data and information contained in those records should reflect the conditions and circumstances that existed at the time the records were created. In the absence of a showing of material error or injustice, there is a reluctance to recommend that those records be changed. While it is understandable the applicant desires to record her current name in her military records, there is not a sufficiently compelling reason for compromising the integrity of the Army's records at this late date. 2. This Record of Proceedings will be filed in her military record to provide clarity and to deal with any confusion that might arise as a result of changes in her name that occurred after she retired. 3. Regarding her address, there is no evidence of error in the addresses entered in her record, and there is no basis for correcting her record to show an address to which she moved many years after she retired. 4. She should, however, update her address with the Defense Finance and Accounting Service, which maintains contact information for military retirees. She may update her information by calling 1-800-321-1080 or by accessing her account online at https://mypay.dfas.mil/mypay.aspx.
