The applicant, a career noncommissioned officer (NCO) in the California Army National Guard, is currently a staff sergeant/E-6 holding military occupational specialty 11B (Infantryman). 2. His Army Military Human Resource Record (AMHRR) shows that he served on active duty: a. as a member of the U.S. Marine Corps from 26 September 1988 to 24 March 1989. His DD Form 214 shows in: (1) item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) the Rifle Expert Badge. (2) item 14 (Military Education) that he completed the Marine Air Ground Task Force Intelligence Specialist (Entry) Course. b. as a member of the Army National Guard on active duty from 5 January 2006 to 15 March 2007. His DD Form 214 shows in: (1) item 13 that he was awarded or authorized the Bronze Star Medal, Army Commendation Medal, National Defense Service Medal, Afghanistan Campaign Medal, Global War on Terrorism Service Medal, Army Service Ribbon, Overseas Service Ribbon, Armed Forces Reserve Medal with M Device, and the NATO Medal. (2) item 14 "NONE." c. as a member of the Army National Guard on active duty from 20 January 2008 to 21 May 2009. His DD Form 214 shows in: (1) item 13 he was awarded or authorized the Afghanistan Campaign Medal with campaign star, Bronze Star Medal, Army Commendation Medal, National Defense Service Medal with bronze star, Global War on Terrorism Expeditionary Medal, Global War on Terrorism Service Medal, Army Service Ribbon, Overseas Service Ribbon (2nd Award), Armed Forces Reserve Medal with M Device, and the NATO Medal. (2) item 14 that he completed the Combat Lifesavers Course, 1 week, 2008, and the Military Transition Team Course, 8 weeks, 2008. d. as a member of the Army National Guard on active duty from 11 November 2010 to 22 February 2012. His DD Form 214 shows in: (1) item 13 he was awarded or authorized the Afghanistan Campaign Medal with four campaign stars, Bronze Star Medal, Army Commendation Medal (2nd Award), Army Achievement Medal, Army Good Conduct Medal, Army Reserve Components Achievement Medal, National Defense Service Medal with bronze service star, Global War on Terrorism Service Medal, Iraq Campaign Medal with campaign star, Army Service Ribbon, Overseas Service Ribbon (3rd Award), Armed Forces Reserve Medal with M Device, NATO Medal, and the Combat Action Badge. (2) item 14 "NONE." (3) item 18 (Remarks) "SERVICE IN IRAQ 20110101-20111107." 3. The applicant provides copies of: a. permanent orders showing he was awarded the: (1) Bronze Star Medal with oak leaf cluster (2nd Award) on 11 April 2009 for his exceptionally meritorious service from 13 April 2008 to 10 April 2009, (2) Combat Infantryman Badge on 28 April 2009; and (3) U.S. Army Excellence in Competition Badge, Bronze, Pistol, on 7 October 2010. The orders show he was recognized for combat excellence in competition during the 2010 California National Guard State Championship. b. certificates showing he completed the: (1) Military Transition Team training for Class #40 from 4 February 2008 to 26 March 2008 at Fort Riley, KS, (2) Iraq Theater of Operations Counter Radio-Controlled Improvised Explosive Device Electronic Warfare Course, 40 hours at Camp Buehring, Iraq, c. DA Form 1059 showing he completed the: (1) CREW Master Gunner Course on 20 August 2010 at Fort Huachuca, AZ; and (2) Equal Opportunity Leader Course on 22 October 2010 at Joint Forces Headquarters, Sacramento, CA 4. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The regulation directs, in pertinent part, that the purpose of the separation document is to provide the individual with documentary evidence of his or her military service at the time of release from active duty, retirement or discharge. It states for: a. item 13: List awards and decorations for all periods of service in the priority sequence specified in AR 600-8-22. Each entry will be verified by the soldier’s records. Do not use abbreviations. b. item 14: List formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 (emphasis added). Include title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills. 5. Army Regulation 600-8-22 (Military Awards) does not list the U.S. Army Excellence in Competition Badge, Bronze, Pistol as a combat or special skill badge for individual wear. DISCUSSION AND CONCLUSIONS: 1. The applicant provided permanent orders showing he was awarded the Bronze Star Medal (2nd Award) and the Combat Infantryman Badge. These awards should be added to his DD Form 214. 2. He provided a certificate showing he completed the Iraq Theater of Operations Counter Radio-Controlled Improvised Explosive Device Electronic Warfare Course, 40 hours at Camp Buehring, Iraq. It would be appropriate to add this training to his DD Form 214 ending 22 February 2012. 3. He provided a certificate showing he completed the Military Transition Team Training from 4 February to 26 March 2008. This training is listed on his DD Form 214 ending 21 May 2009. A correction to his record is not necessary. 4. He provided DA Forms 1059 showing he completed the CREW Master Gunner Course on 20 August 2010, and the Equal Opportunity Leader Course on 22 October 2010. There is no available evidence that he was on active duty during a period covered by a DD Form 214 when he completed these training courses. A correction to his records is not authorized. 5. He provided orders showing he was awarded the U.S. Army Excellence in Competition Badge, Bronze, Pistol. There is no available evidence showing this badge is authorized to be entered on his DD Form 214. 6. His Marine Corps Intelligence Specialist Course completed in March 1989 is listed on his DD Form 214 ending 24 March 1989. A correction to his record is not necessary. The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. adding to item 13 of his DD Form 214 ending 22 February 2012 the: * Bronze Star Medal (2nd Award) * Combat Infantryman Badge b. adding to item 14 of his DD Form 214 ending 22 February 2012 the Iraq Theater of Operations Counter Radio-Controlled Improvised Explosive Device Electronic Warfare Course, 40 hours. 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief.
