Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Joint Meritorious Unit Award. 2. The applicant states, in effect, he served in Iraq while assigned to the Gulf Region District, U.S. Army Corps of Engineers. This unit was cited for award of the Joint Meritorious Unit Award during the period 21 October 2009 to 11 February 2010. He served as the Deputy, Project and Program Management, while assigned to Camp Victory, Iraq. He was told by fellow officers who know the Joint Meritorious Unit Award was sent through the Gulf Region District Headquarters and the Multi-National Corps-Iraq G-1 to the Multi-National Force-Iraq G-1. He does not have the ability to locate the award certificate since he is retired from the U.S. Army Reserve (USAR) and no longer has a common access card. 3. The applicant provides no additional evidence. CONSIDERATION OF EVIDENCE: 1. The applicant's records show he was a member of the USAR and he was assigned to the 416th Engineer Command, Darien, IL. He entered active duty on 6 November 2008 in support of Operation Iraqi Freedom as a member of his USAR unit. He served in Iraq from 7 December 2008 to 17 February 2010 while assigned to Headquarters, Multi-National Corps-Iraq. 2. Headquarters, Multi-National Corps-Iraq, Permanent Order 365-047, dated 31 December 2009, awarded him the Defense Meritorious Service Medal for meritorious service during the period 8 December 2008 to 11 February 2010 while assigned to Multi-National Corps-Iraq. 3. He was honorably released from active duty on 13 April 2010. The DD Form 214 he was issued at the time of his release from active duty shows he completed 1 year, 5 months, and 7 days of creditable active service during this period. 4. Department of the Army General Orders Number 2010-08, dated 22 July 2010, cited Headquarters, Multi-National Corps-Iraq, for award of the Joint Meritorious Unit Award for meritorious service during the period 19 February 2008 to 4 April 2009. 5. His records contain a DD Form 215 (Correction to DD Form 214), dated 16 July 2012, that added award of the Defense Meritorious Service Medal to his DD Form 214 for the period ending 13 April 2010. 6. His records also contain a DD Form 215, dated 10 January 2013, that deleted award of the Iraq Campaign Medal with campaign star and added award of the Iraq Campaign Medal with two bronze service stars to his DD Form 214 for the period ending 13 April 2010. 7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for preparation of the DD Form 214 and DD Form 215. The DD Form 214 provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. Once a DD Form 214 has been issued, do not reissue, except when two DD Forms 215 have been issued and an additional correction is required. DISCUSSION AND CONCLUSIONS: 1. General orders awarded the applicant's unit the Joint Meritorious Unit Award for meritorious service during the period he was assigned to the unit which is not shown on his DD Form 214. Therefore, it would be appropriate to correct his DD Form 214 for the period ending 13 April 2010 to show this unit award. 2. He has already been issued two DD Forms 215 for the period ending 13 April 2010. These DD Forms 215, dated 16 July 2012 and 10 January 2013, and his DD Form 214 for the period ending 13 April 2010 should be voided and a new DD Form 214 should be issued to show award of the Joint Meritorious Unit Award in addition to all of the information currently shown on the previously-issued forms.
