Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his retirement award and an education certification be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states the supporting documents weren't available at the time of his separation. 3. The applicant provides copies of his DD Form 214; a certificate designating him a "Demonstrated Master Logistician; a letter, dated 30 November 2012; the certificate with orders number for the Legion of Merit; and his Officer Record Brief (ORB). CONSIDERATION OF EVIDENCE: 1. The applicant retired from the Regular Army as a major on 31 January 2013. 2. His DD Form 214 does not list the Legion of Merit and block 14 (Military Education) does not show his Demonstrated Master Logistician certification. 3. Permanent Orders Number 17-002, dated 17 January 2013, issued by Headquarters, U.S. Army South awarded the applicant the Legion of Merit for meritorious service over a 22 year career. 4. Section VI – Military Education of the applicant's ORB shows, "DEMONSTRATED MSTR LOGI 2012." 5. The applicant submitted: a. The certificate designating him a "Demonstrated Master Logistician." It states that his "professional qualifications and achievements" have been examined but does not identify an associated course of study; and b. The 20 November 2012 letter from the Commandant/Military Deputy, Army Logistics University states that the Demonstrated Master Logistician designation should be placed on the ORB in Section VI – Military Education. 6. Army Regulation 635-5 (Separation Documents) provides the instructions for completing the DD Form 214. The specific directions for block 14? Military Training state, "From ERB/ORB, list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214. Include title, length in weeks, and year completed. 7. The Demonstrated Master Logistician (DML) Program has three tracks. Track One includes two events from the DML Lifelong Learning Table; experience, to include 5 years field experience; an undergraduate degree from an accredited college or university; and meeting the requirements for the previous level. Track Two further includes one of two logistics executive development courses. Track Three further includes completion of a National Logistics Curriculum Master of Science from one of several sponsoring institutions. DISCUSSION AND CONCLUSIONS: 1. The applicant was awarded the Legion of Merit and it should be added to his DD Form 214. 2. The Demonstrated Master Logistician is a program, not one training course. While it appears it was appropriately added to his ORB it does not appear to be an authorized entry on the DD Form 214. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Legion of Merit to his DD Form 214 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
