The applicant states the middle initial shown on the orders is not his. 3. The applicant provides: * his DD Form 214 (Certificate of Release or Discharge from Active Duty) * Permanent Orders 276-027, issued by Headquarters, 143rd Sustainment Command (Expeditionary) Afghanistan, dated 3 October 2009 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 1 October 2008. He was retired due to temporary disability effective 19 September 2011. His DD Form 214 shows his middle name as "Vernon." The Combat Action Badge is listed among awards listed on his DD Form 214. 2. Permanent Orders 276-027, dated 3 October 2009, awarded the applicant the Combat Action Badge for service on 18 August 2009. These orders show his middle initial as "B." 3. His record contains an Enlisted Record Brief that shows his middle name as "Vernon."
