THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, reconsideration of her earlier request for correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal (4th Award). 2. The applicant states Permanent Order Number 238-002 was never added to her records and she did not know to check her records for accuracy as a young Soldier. 3. The applicant provides four DA Forms 638 (Recommendation for Award). CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20120013664 on 26 February 2013. 2. The applicant provides a DA Form 638 which was not previously considered by the Board. This is new evidence that warrants consideration by the Board. 3. The applicant enlisted in the Regular Army on 8 September 2005 and she held military occupational specialty 15D (Aircraft Powertrain Repairer). 4. Her records contain: * Permanent Order Number 115-007, undated, issued by Headquarters (HQ), 25th Combat Aviation Brigade, Schofield Barracks, HI, awarding her the Army Commendation Medal for exceptionally meritorious service from 15 May 2006 to 1 May 2009 * Permanent Order Number 277-118, dated 4 October 2011, issued by HQ, 77th Theater Aviation Brigade, Iraq, awarding her the Army Commendation Medal for exceptionally meritorious service from 28 February to 31 December 2011 * Permanent Order Number A-129-2, dated 8 May 2012, issued by HQ, 21st Cavalry Brigade, Fort Hood, TX, awarding her the Army Commendation Medal with 3rd Oak Leaf Cluster for exceptionally meritorious service from 1 June 2009 to 7 June 2012 as an expiration of term of service award 5. She was honorably discharged on 7 June 2012 by reason of completion of required active service. She completed 6 years and 9 months of total active military service. 6. Her DD Form 214 shows award of the Army Commendation Medal (3rd Award). 7. She provides a DA Form 638 showing Permanent Order Number 238-002, dated 26 August 2007, issued by HQ, 2nd Squadron, 6th Cavalry Regiment, Kirkuk, Iraq, awarded her the Army Commendation Medal with 1st Oak Leaf Cluster for meritorious service in support of Operation Iraqi Freedom for the period 25 July 2006 to 31 July 2007. DISCUSSION AND CONCLUSIONS: The applicant provides orders showing entitlement to four awards of the Army Commendation Medal; therefore, she is entitled to correction of her DD Form 214 to show award of the Army Commendation (4th Award).
