Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of the DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) pertaining to her deceased husband, a former service member (FSM), to reflect all of his service. 2. The applicant states, in effect, that the FSM told her before he passed away that his DD Form 214 did not reflect all of his service. 3. The applicant provides a copy of a Record of Proceedings obtained from the internet pertaining to another service member. CONSIDERATION OF EVIDENCE: 1. The FSM enlisted in the Regular Army in Atlanta, Georgia on 5 November 1965 for a period of 3 years. He completed his basic training at Fort Benning, Georgia and his advanced individual training as a light weapons infantryman at Fort Dix, New Jersey and was transferred to Fort Devens, Massachusetts on 4 April 1966 for his first assignment. He was assigned to Company B, 3rd Battalion, 21st Infantry Regiment, 196th Light Infantry Brigade. 2. On 15 July 1966, he was transferred to Vietnam with his unit and on 19 February 1967 he was wounded in action (fragmentation wound to the right eye) during a search and destroy mission. He was evacuated to the 3rd Field Hospital in Saigon. 3. He was evacuated to a hospital at Camp Zama, Japan on 6 April 1967 where he remained for 10 days before being transferred to the hospital at Fort Gordon, Georgia where his eye was removed. 4. He was then transferred to Walter Reed General Hospital on 3 June 1967 for fitting of a prosthesis and review by a medical board. 5. On 14 November 1967, he was honorably retired by reason of physical disability with a 40 percent disability rating. He had served 2 years and 10 days of active service. 6. A review of his official records fails to reveal any other service performed by the FSM that is not reflected on his DD Form 214. 7. Army Regulation 635-5 (Separation Documents) serves as the authority for the preparation of the DD Form 214. It states that data used to complete the DD Form 214 will be obtained from official records at the time of separation. DISCUSSION AND CONCLUSIONS: 1. The applicant’s contentions and supporting documents have been noted and appear to lack merit. 2. The FSM’s official records clearly show that he enlisted on 5 November 1965, served 2 years and 10 days of active service, and was honorably retired by reason of physical disability on 14 November 1967. 3. Therefore, in the absence of evidence to show otherwise, there appears to be no basis to grant the applicant’s request.
