THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the: * Global War on Terrorism Expeditionary Medal (GWOTEM) * Meritorious Unit Commendation (MUC) * Army Achievement Medal (AAM) (5th Award) 2. The applicant states he was deployed to Kuwait from 23 June 2011 to 21 May 2012 and should have been awarded the GWOTEM. His unit, the 719th Movement Control Battalion (MCB), was awarded the MUC for its deployment from 24 June 2007 to 6 April 2008. He was awarded a total of five AAM's, but only four of these awards are shown on his DD Form 214. 3. The applicant provides copies of: * five AAM Certificates * Orders 07-094-00013, 94th Regional Readiness Command, dated 4 April 2007 * DD Form 214 for the period ending 9 May 2008 * memorandum, U.S. Army Human Resources Command (HRC), dated 21 December 2009 * Permanent Orders 224-24, HRC, dated 12 August 2009 * memorandum, U.S. Army Center of Military History, dated 17 March 2010 * Orders 10-362-00012, Deployment Support Command, 28 December 2010 * Orders CS-172-0071, Camp Shelby Joint Forces Training Center, dated 21 June 2011 * DD Form 214 for the period ending 26 July 2012 CONSIDERATION OF EVIDENCE: 1. At the time of his application, he was serving in the U.S. Army Reserve (USAR) in the rank/grade of captain/O-3. 2. The following DA Forms 4980 (AAM Certificate) provided by the applicant and also filed in his Army Military Human Resource Record (AMHRR) show he was awarded the AAM for exceptionally meritorious service or achievement for the periods indicated: * for significant contributions to the unit's combat-ready posture during the period 3 November 1982 through 21 September 1983 * for outstanding performance of duty during annual training from 14 to 29 July 1989 * for outstanding professional ability and energetic approach while participating in the Memorial Day Parade * for contributions during Cyclone Thrust Warfighter from 16 to 26 January 2007 * for participation in the 316th Sustainment Command (Expeditionary) Logistics Training Exercise during the period 18 to 31 March 2007 3. 94th Regional Readiness Command Orders 07-094-00013, dated 4 April 2007, provided by the applicant, ordered him to active duty as a member of the 719th Transportation Battalion, Detachment 2, USAR, effective 1 April 2007 for a period of 532 days for the purpose of mobilization in support of Operation Iraqi Freedom. 4. His DD Form 214 for this period shows he entered active duty on 13 April 2007 and he was released from active duty (REFRAD) on 9 May 2008. His DD Form 214 also shows: a. his unit of assignment as the 719th Transportation Battalion, Detachment 2; b. his service in Iraq from 24 June 2007 to 6 April 2008; and c. his authorized awards as the: * Meritorious Service Medal (2nd Award) * Army Commendation Medal * AAM (4th Award) * Army Reserve Components Achievement Medal * National Defense Service Medal (2nd Award) * Global War on Terrorism Service Medal * Iraq Campaign Medal * Noncommissioned Officer Professional Development Ribbon * Army Service Ribbon * Overseas Service Ribbon * Armed Forces Reserve Medal with "M" Device and 20-Year Device * Driver and Mechanic Badge with Mechanic Clasp 5. An HRC memorandum, dated 21 December 2009, and HRC Permanent Orders 224-24, dated 12 August 2009, announced award of the MUC to Headquarters and Headquarters Detachment (HHD), 719th Transportation Battalion, and its subordinate units for the period 24 June 2007 to 5 April 2008. 6. On 8 June 2011, the applicant was ordered to active duty as a member of the USAR in support of Operation New Dawn. He was REFRAD on 26 July 2012. His DD Form 214 for this period shows: a. his unit of assignment with the U.S. Central Command; b. his service in Kuwait from 23 June 2011 to 21 May 2012; and c. his authorized awards as the: * Armed Forces Reserve Medal with "M" Device and 20-Year Device (Silver Hourglass) * Meritorious Service Medal (3rd Award) * Army Commendation Medal * Army Achievement Medal (4th Award) * Army Reserve Components Achievement Medal (2nd Award) * National Defense Service Medal (2nd Award) * Global War on Terrorism Service Medal * Iraq Campaign Medal with one bronze service star * Noncommissioned Officer Professional Development Ribbon * Army Service Ribbon * Overseas Service Ribbon (2nd Award) * Driver and Mechanic Badge with Mechanic Clasp 7. Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. a. The GWOTEM is authorized for award to members of the Armed Forces of the United States who deployed abroad for service in Global War on Terrorism operations on or after 11 September 2001 to a date to be determined. b. Initial award of the GWOTEM was limited to service members who deployed abroad in support of Operations Enduring Freedom and Iraqi Freedom in designated specific geographic areas of eligibility, including Kuwait. DISCUSSION AND CONCLUSIONS: 1. The applicant contends that his DD Form 214 for the period ending 26 July 2012 should be corrected to show award of the GWOTEM, MUC, and AAM (5th Award). 2. The available evidence clearly shows HHD, 719th Transportation Battalion, and its subordinate units were awarded the MUC during the period the applicant was in Iraq. Accordingly, his DD Form 214 for the period ending 26 July 2012 should be corrected to show award of the MUC. 3. The available evidence shows the applicant served a qualifying period of duty in Kuwait for award of the GWOTEM. Therefore, his DD Form 214 for the period ending 26 July 2012 should be corrected to show award of the GWOTEM. 4. The available evidence clearly shows the applicant was awarded a total of five AAM's, but his DD Form 214 for the period ending 26 July 2012 only reflects a total of four AAM's. Therefore, his DD Form 214 for the period ending 26 July 2012 should be corrected to show award of the AAM (5th Award).
