THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal.

2.  The applicant states she received the Army Commendation Medal after her discharge. 

3.  The applicant provides a copy of her Army Commendation Medal Certificate.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 26 October 2009.  She completed training and was awarded military occupational specialty 91B (Wheeled Vehicle Mechanic).

2.  Permanent Order Number 249-04, issued by Headquarters, 101st Sustainment Brigade, 101st Airborne Division, Fort Campbell, KY, dated 5 September 2012, awarded the applicant the Army Commendation Medal.

3.  On 2 December 2012, the applicant was honorably released from active duty and transferred to the U.S. Army Reserve Control Group (Reinforcement).  Her DD Form 214 shows she completed 3 years, 1 month, and 7 days of creditable active military service.  Her DD Form 214 also shows award of the National Defense Service Medal, Global War on Terrorism Service Medal, and Army Service Ribbon.
DISCUSSION AND CONCLUSIONS:

Permanent orders awarded the applicant the Army Commendation Medal; therefore, she is entitled to correction of her DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 to show award of the Army Commendation Medal.
