THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests removal of the highlighted statements recorded on his DA Form 4707 (Entrance Physical Standards Board (EPSBD) Proceedings). 2. The applicant states he did not tell the truth at the time the form was prepared. 3. The applicant provides a highlighted DA Form 4707. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 25 June 2012 for a period of 6 years and training as an Explosive Ordnance Disposal Specialist. He was transferred to Fort Jackson, South Carolina, to undergo basic training. 2. On 6 July 2012, an EPSBD was convened and determined the applicant suffered from a depressive disorder, alcohol abuse, and cannabis abuse. The board recommended his separation for failure to meet medical procurement standards. The applicant concurred with the findings and recommendations of the board and requested to be discharged. The findings were approved on 11 July 2012. 3. Accordingly, he was discharged on 31 July 2012 under the provisions of Army Regulation 635-200 (Active Duty Enlisted Administrative Separations), paragraph 5-11, due to failure to meet medical/physical procurement standards. He completed 1 month and 6 days of active service and his service was uncharacterized. 4. The highlighted DA Form 4707 provided by the applicant shows the applicant reported a history of alcohol and drug abuse, depression, thoughts of suicide, and a verbally-abusive family life. DISCUSSION AND CONCLUSIONS: 1. The applicant's contentions and supporting documents have been noted and appear to lack merit. 2. The information recorded on the DA Form 4707 was obtained from the applicant at the time of his EPSBD Proceedings. He failed to provide any evidence to show the statements on the form are in error or are untrue. 3. While the applicant admitted that he was lying at the time, he has not provided any means in which to ascertain that he is now telling the truth. 4. Accordingly, there appears to be no basis to grant his request.
