THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 2 (Department, Component, and Branch) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) ending on 8 May 2012. 2. The applicant states item 2 of his 2012 DD Form 214 should list the entry, "ARMY/USAR (U.S. Army Reserve)" and not "ARMY/RA (Regular Army)." He was a member of the USAR during the period covered by this DD Form 214. He entered on active duty for a deployment as an Army Reserve Soldier. He was not an active duty Soldier. 3. The applicant provides copies of the following: * 2010 DD Form 4/1 (Enlistment/Reenlistment Document – Armed Forces of the United States) * Orders Number 11-044-00027, 11-061-00100, and PK-133-0003 * North Atlantic Treaty Organization (NATO) Travel Orders * 2012 Leave and Earnings Statement (LES) * 2012 DD Form 214 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the USAR for 8 years on 11 June 2002. He reenlisted in the USAR on 29 May 2008 for 3 years and on 21 December 2010 for 6 years. 2. He provided copies of the following: a. Orders Number 11-044-0027 and 11-061-00100, issued by the 95th Training Division, on 13 February and 2 March 2011, ordering him to active duty as a member of the USAR in support of Operations Enduring Freedom (OEF), with a report date of 3 March 2011 for a period of 400 days. Both orders show his component as "USAR." b. Orders Number PK-133-0003, issued by Headquarters, Joint Readiness and Training Center, Fort Polk, LA, on 13 May 2011, deploying him Afghanistan with a proceed date of 15 May 2011. c. NATO Travel Orders pertaining to his travel from the United States to Afghanistan with an expected departure date of 5 April 2011 and an expected return date of 5 April 2012. d. An LES, dated 15 February 2012, which he marked to indicate he received hostile fire pay. The LES also shows his branch as "USAR." 3. He was honorably released from active duty on 8 May 2012. His DD Form 214 lists in: * Item 2, "ARMY/RA" * Item 12a (Date Entered Active Duty This Period), 3 March 2011 * Item 18 (Remarks), Service in Afghanistan from 24 May 2011 through 29 February 2012 and Ordered to Active Duty in Support of OEF 4. Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214. It states the DD Form 214 will be prepared for all personnel at the time of their retirement, discharge, or release from active duty. The regulation states item 2 will list for officers and enlisted Soldiers their branches as following: "RA," "Army National Guard," or "USAR." DISCUSSION AND CONCLUSIONS: The evidence of record shows the applicant enlisted and reenlisted in the USAR. He was ordered to active duty as a member of the USAR and entered active duty on 3 March 2011. He was honorably released from active duty on 8 May 2012. It appears his DD Form 214 inadvertently listed his component as "RA" vice "USAR." Therefore, he is entitled to correction of item 2 of his DD Form 214 to list the entry, "ARMY/USAR."
