THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of items 19a (Mailing Address After Separation) and 19b (Nearest Relative) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) ending on 30 January 2013. 2. The applicant states his home address and telephone number need to be corrected. The Department of Veterans Affairs and others are still sending mail to his old address on Fort Belvoir, VA. He has written in his new address in Huntsville, AL, and telephone number in items 19a and 19b of this DD Form 214. When he retired he went on terminal leave and still resided on Fort Belvoir. He has since relocated to AL and his new address should be a forever address. 3. The applicant provides two copies of his 2013 DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 17 May 1987. He served continuously on active duty through various assignments and reenlistments until he was honorably retired on 30 January 2013. His DD Form 214 list in items 19a and 19b the same address of XXXX Mason Drive, Fort Belvoir, VA. This DD Form 214 was electronically signed by him on 12 October 2012. 2. His records contain the following: * A DD Form 93 (Record of Emergency Data), dated 1October 2012, which lists the same address as items 19a and 19b of his DD Form 214 * A DD Form 2656 (Data for Payment of Retired Personnel), dated 31 January 2013, wherein he listed his correspondence address as XXXX Mason Drive, Fort Belvoir, VA 4. Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214. It states the DD Form 214 would be prepared for all personal at the time of their retirement, discharge, or release from active duty. The regulation states: * Item 19a - will be provided by the Soldier and must be a permanent address * Item 19b - will list the name and address of a relative and should be someone who will know their location and address at all times, there are no provisions for changing this address at a later date for the convenience of the Soldier DISCUSSION AND CONCLUSIONS: Regulatory guidance does not provide for correction of items 19a and 19b of the DD Form 214 at a later date for the convenience of the Soldier. At the time of his separation, he provided the address indicated on his DD Form 214. There is no evidence of record and he provided none to show items 19a and 19b his 2013 DD Form 214 contain an error. In view of the foregoing, he is not entitled to the requested relief.
