THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Army Good Conduct Medal (AGCM) and the correct period of his deployment. 2. The applicant states: a. His AGCM (1st Award) was omitted from his DD Form 214. b. The period of his oversea deployment was amended to shorten his tour and his DD Form 214 should be corrected to show this service was from 14 February to 5 November 2012. 3. The applicant provides: * DD Form 214 * AGCM Order * Enlisted Record Brief (ERB) * Deployment Order * Release from Active Duty (REFRAD) Order * Amendment Order CONSIDERATION OF EVIDENCE: 1. The applicant’s military record shows he enlisted in the Regular Army on 27 January 2010. He was trained in and awarded military occupation specialty 91D (Power Generation Equipment Repairer). 2. United States Army Intelligence Center of Excellence, Fort Huachuca, published the following orders as indicated: a. Order Number (#) HU-313-0069, dated 9 November 2011, which ordered his deployment to Bagram, Afghanistan, in support of Operation Enduring Freedom (OEF), for 365 days with a proceed date of 5 January 2012; and b. Order #HU-313-0069 (A2), dated 9 February 2012, which amended Order #HU-313-0069, dated 9 November 2011, to change the “Proceed Date” from 5 January 2012 to 14 February 2012. 3. On 20 February 2013, Headquarters, 11th Theater Tactical Signal Brigade, Fort Huachuca, published Permanent Order #044-02. It announced the applicant’s AGCM (1st Award) for the period 27 January 2010 to 26 January 2013. 4. His ERB shows in Section I (Assignment Information) that he performed 12 months oversea/deployment combat duty in Afghanistan 5 January 2012 to 5 January 2013. It also list the AGCM in Section VIIII (Awards and Decorations). 5. On 26 May 2013, the applicant was honorably REFRAD after completing 3 years and 4 months of creditable active duty service. The DD Form 214 issued at that time shows: * Item 12f (Foreign Service) – "0000 06 03" * Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) – does not list the AGCM * Item 18 – "SERVICE IN AFGHANISTAN 20120429 – 20121101" 6. A review of the applicant's Defense Finance and Accounting Service (DFAS) military pay record shows he received HFP/IDP and CTZE for service in Afghanistan from 17 February through 2 November 2012. 7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents which are prepared for individuals upon retirement, discharge, or release from active military service or control of the Army. It also establishes standardized policy for preparing and distributing the DD Form 214. It states for: a. Item 12f, enter the total amount of foreign service completed during the period covered by the DD Form 214; b. item 13, enter awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22 (Military Awards); and c. item 18, for an active duty Soldier deployed with his or her unit during their continuous period of active service covered by the DD Form 214, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." DISCUSSION AND CONCLUSIONS: 1. The evidence of record confirms the applicant received the AGCM. It is listed on his ERB and this award was announced in official orders. This award is not currently listed in item 13 of his DD Form 214 and it should be added at this time. 2. The evidence of record includes amendment orders confirming the applicant's initial period of deployment to Afghanistan was changed. However, this period of oversea service is listed differently on his ERB, DD Form 214, and DFAS record, confirming the existence of this error. Accordingly, items 12f and 18 of his DD Form 214 should be corrected as he claims to accurately reflect his 8 months and 22 days foreign service in Afghanistan from 14 February to 5 November 2012.
